中文摘要 | 地中海型貧血是一種因無效紅血球細胞生成致需多次輸血之次發性血色素相關疾病。肝活體切片檢查是主要的診斷方法,但切片檢查是屬侵入性檢查且容易出現採樣誤差,難以重複實施採樣。因此非侵入性的體內肝臟鐵量化工具如磁振造影(MR)和雙能量電腦斷層掃描(DECT)成為研究之主軸。在過去DECT必須單獨進行不同的能量影像掃描,因此需有較長之影像後處理時間及具有影像取樣分析不同步之盲點。最近推出的雙源雙能(DSDE)電腦斷層技術已經允許掃描時同時使用兩個球管同時取像,增加臨床應用之有效性。本研究的目的是利用鐵含量假體評估雙源雙能量電腦斷層量化肝臟和心臟鐵沉積之可能性,同時並比較其與1.5T磁振造影影像之相關性。鐵含量假體研究結果顯示,雙源雙能量電腦斷層影像所得到之ΔHU與鐵濃度之增加具有高度正相關(R^2>0.95)。本研究顯示雙源雙能量電腦斷層所得到之影像與磁振造影影像所得到之結果,具有良好的相關性。因此推論雙源雙能量電腦斷層可準確量化鐵沉積的濃度,成為臨床診斷另一個有利之工具。 |
英文摘要 | Thalassemia is one kind of secondary hemochromatosis related to ineffective erythropoiesis and multiple transfusions. Hepatic biopsy is considered the reference standard method for diagnosis but it is invasive, prone to sampling error, and difficult to repeat. Therefore, there have been attempts at noninvasive in-vivo methods for hepatic iron quantification, such as magnetic resonance (MR) imaging, and dual-energy (DE) computed tomography (CT). In the past, DECT had to be separately performed with sequential scanning at different energies, inevitably leading to redundant post-processing time and spatial and temporal mis-registration artifacts. The recently introduced dual-source dual-energy (DSDE) CT technique has allowed the performance of DE scanning with simultaneous use of two tube-detector combinations, extending its clinical usefulness. Therefore, the purpose of our study was to assess the feasibility of DSDE CT for the evaluation of iron accumulation in liver and heart phantoms and to compare its correlation with that of 1.5T MR imaging. The results of our phantom study show that ΔHU deviated from DSDE CT images was nearly perfectly correlated with the iron concentration(R^2>0.95). Our study also shows that DSDE CT exhibits similar excellent correlation to iron phantom as MR imaging. In addition, DSDE CT can be used as an accurate modality for diagnosing clinically important hepatic iron accumulation. |
