| 中文摘要 | 唾液腺結石常會引起病人臉頰腫脹、嘴巴酸痛、吞嚥困難,甚至會有局部或全身發炎或感染反應,而影響進食與生活品質。臨床上多為高度懷疑唾液腺結石再由耳鼻喉科轉介至牙科進行攝影。傳統咬合攝影(Occlusal radiography)擺位法或環口放射攝影(Panoramic radiography)會因為先天解剖構造限制影像的診斷,而增加誤診的可能性或忽略了病灶成因,根據病人主訴區域使用改良式的下頷後斜位咬合攝影法可避開大部分的口腔顱顏顎面骨骼,使半鈣化或微結石在軟組織或口底顯現。本次研究使用牙科專用數位口內放射攝影4號咬合片,可以使病人在最短時間、最少影像張數、同時劑量僅約5~8 μSv,為最簡單有效的術前診斷方式。此方法亦比較同病人採用傳統與改良式攝影的影像結果,以及環口放射攝影與下頷橫截面咬合攝影法之結石位置影像;在符合合理抑低的精神下,不用過多的二維照射影像或其他非必須之輻射暴露及顯影劑使用,提供一客觀攝影技術以供參考。 |
| 英文摘要 | Facial swelling, pain and hard swallowing are the classic symptoms caused by sialolithiasis, even have local inflammation or infection sometimes that affect the eating and life quality of patient. Clinically, most of patients transfer to dentistry radiology by ENT whom highly suspected salivary calculus. Traditional occlusal radiography or panoramic radiography images will limit by anatomic structure, which increase the misdiagnosis probability or ignore the reason cause lesions. However, use modified submandibular lower oblique occlusal radiography according to the complaint of patients could avoid sialolithiasis hidden from oral-maxillofacial skeleton. Moreover, this technique made present half-calcified or semi-stone could be shown in radiography. 4 series intraoral digital occlusal PSP would be used in this study. It’s simplest effectively diagnosis for preoperative evaluation because of less time consuming, less radiology images and only 5~8 μSv radiation dose. Furthermore, the result compared traditional occlusal radiography and panoramic radiography with modified occlusal radiography in same patient for example respectively. For the spiritual of ALARA, we recommend an objective radiography substitute excessive plain film or any other unnecessary radiation exposure with contrast agent. |
