| 中文摘要 | 床邊攝影是利用移動式X光機所完成的攝影驗查,主要是提供移動困難或檢查過程無法配合例如加護病房及急診、病房重大傷病的患者檢查上的便利,面對此類無法主動配合的受檢者時,放射師必需尋求護理人員或家屬的協助及利用輔助工具,擺妥適當攝影姿勢,由於擺位困難度高,所以重照的機率也相對地提高;本研究目的是要分析床邊攝影重照原因及類型,提出改善措施,降低床過攝影重照率,提升受檢者安全;本研究是利用數位移動式X光機的QC軟體系統統計2012年1月至12月床邊攝影重照數及重照原因並進行原因分析,發現重照率達6.99%,一半的原因為影像邊緣切到,因此改善影像切到對於降低床邊攝影重照率會有顯著的幫助。針對影像切到,使用三項改善對策:繼續教育、調閱舊影像,以及影像接收板(Imaging Plate,簡稱IP)加上輔助設備,四周貼上3 mm的泡棉雙面膠,收集2013年1月至2013年12月共12個月每月床邊攝影的重照率,床邊攝影自6.99%降為3.04%,其中影像切到自3.75%降到1.86%,利用卡方檢定分析(Chi-Square),統計分析數據顯示,床邊攝影在改善前後的總重照率的比較,其p值為0. 002(小於0.05),顯示改善後總重照率p值達顯著的水準,其中改善前後的影像切到重照率的比較,p值為0.049,也小於0.05,具統計上的意義;綜合以上的分析結果,表示改善對策能有效地降低床邊攝影重照率,提升受檢者的安全及改善醫療照護品質。 |
| 英文摘要 | Portable x-ray is a photographic examination performed with a mobile x-ray machine. It offers convenience for patients who have difficulty moving or cannot cooperate during procedure, such as intensive care unit (ICU) patients, and patients in emergency room. When facing a patient who cannot cooperate or needs assistance during the examination, a radiographer usually need help from others or need to use auxiliary equipment. Under this kind of situation, the chance of getting an imperfect image is much higher, compared with the ideal situation when a patient would cooperate voluntarily. This project aims to lower portable x- ray retake rates and improve safety for patients under the procedure by analyzing reasons and types of retakes. We made use of quality control (QC) software system of digital radiography (DR) portable machine, counted and analyzed all retakes and categorized their reasons from January through December 2012. We found retake rates was 6.99% and half of reason is cutting to image edge, thus improving this factor to reduce retaking rate will help significantly. To overcome the problem of cutting to image edge, we came up with three improvement measures: (1) continued education program for radiographers, (2) review of previous images, (3) improving imaging plate (IP) for image receiving fixture by affixing 3mm double-sided adhesive foam around the edge of it. We collected data of monthly retake rates from January through December 2013, and analyzed the data with Chi-Square. The result shows that retake rate had dropped from 6.99% to 3.04%, with cause of cutting to image edge dropped from 3.75% to 1.86%. Based on the statistical analysis, the p value was found to be 0.002 (lower than 0.05) when comparing retake rates before and after improvement measures. It shows significant improvement on reducing retake rates. And the p value was 0.049 (lower than 0.05) when comparing retake rates with the cause of cut to image edge before and after improvement measures. The statistic results also indicate that the three improvements measures all contribute to the improvement on retake rates in general. The results confirm that our counter measures have taken effect in lowering portable x-ray retake rates significantly. And it achieves our goal of improving patient safety and providing better health care quality. |
