中文摘要 | 含釓的化合物對腎功能不全病患會有發生腎性全身纖維化症之風險,故本研究以胸腰椎外傷壓迫性骨折為主,評估利用T2加權頻譜預飽和反轉恢復脈衝序列代替施打顯影劑後的T1加權頻譜預飽和反轉恢復脈衝序列的可能性,降低此類患者注射顯影劑之風險。回顧性分析2011年6月至2012年5月21位外傷性胸腰椎壓迫性骨折患者之MRI影像,測量T2加權頻譜預飽和反轉恢復脈衝序列與T1加權頻譜預飽和反轉恢復脈衝序列高訊號區訊號強度與面積進行分析。結果顯示T2加權頻譜預飽和反轉恢復脈衝序列影像訊雜比略低於T1加權頻譜預飽和反轉恢復脈衝序列影像,卻有著較高的對比值(P<0.05);而二組影像在高訊號區的面積差異平均為7.6%±6,相關性達0.991(P<0.001),顯示二組影像的高訊號區面積有著相當高的一致性。因此當面對腎功能不佳的外傷性脊椎壓迫性骨折患者時,在不適合進行顯影劑注射的情況下,適時提供T2加權頻譜預飽和反轉恢復脈衝序列影像確實有助於臨床之診斷意義。 |
英文摘要 | Renal insufficiency patients will induce a nephrogenic systemic fibrosis due to gadolinium-containing compounds. The study assesses traumatic thoracolumbar spine compression fracture patients' possibilities to switch contrast media T1WI spectral presaturation with inversion recovery (SPIR) pulse sequence to T2WI-SPIR pulse sequence to reduce the risk of injection. The retrospective study analysis twenty-one traumatic thoracolumbar spine compression fracture patient's MRI images to measure the intensity and area's signal from high signal area on June 2011 to May 2012. The Results show that the images of T2WI-SPIR signal to noise Ratio was slightly below those in T1WI-SPIR with higher contrast Ratio values (P <0.05); the different averages between two images in high signal area is 7.6% ± 6, and the correlation of the second group of up to 0.991 (P <0.001) which show highly consistency in two areas . When renal insufficiency patients have to take a deal with traumatic thoracolumbar spine compression fracture in inappropriate injection of contrast medium, to use T2WI-SPIR image do help the clinical diagnosis significance. |
