中文摘要 | 本實驗主要利用假體測量頭頸部病人強度調控放射治療之皮膚劑量,以及找出治療計畫所產生的處方劑量(prescribed dose, Dp)之等劑量曲線與表面距離之間的關係。本實驗採用薄片型熱發光劑量計以及輻射變色軟片來做為量測工具,並分別做其特性研究;在IMRT head/neck phantom和anthropomorphic Rando phantom測量模擬的頸部淋巴區的表面劑量,利用此兩種不同的假體以及Eclipse的治療計畫系統製作處方劑量為180 cGy的7個照野治療計畫,進行Dp等劑量曲線到表面距離以及表面劑量之測量。熱發光劑量計與輻射變色軟片的量測值,在假體模擬方面(腫瘤輪廓內縮3 mm),兩者表面劑量的平均量測值誤差小於5 %,當Dp為180 cGy時IMRT head/neck phantom表面劑量的右側量測值,兩個量測工具的平均值對Dp之百分比為56.94 %、56.23 %、52.69 %、55.25 %、57.70 %、54.46 %、52.63 %和53.74 %;左側量測值,兩個量測工具的平均值對Dp之百分比為63.73 %、59.68 %、55.37 %、62.13 %、61.87 %、57.71 %、62.67 %和60.91 %。Rando® phantom表面劑量的右側量測值,兩個量測工具的平均值對Dp之百分比為56.62 %、59.11 %、66.53 %、61.87 %、62.83 %和67.02 %;左側量測值,兩個量測工具的平均值對Dp之百分比為64.48 %、63.25 %、64.74 %、67.31 %、68.56 %和66.54 %。由本實驗得知,表面劑量與處方劑量曲線和表面之間的距離有關,距離越近表面劑量越高,可利用處方劑量曲線和表面之間的距離來評估表面劑量,但實際上表面劑量需要更多實驗分析與統計。 |
英文摘要 | This research was focused on the skin dose of simulated phantoms and the relationship between the distances from isodose curve of prescribed dose (Dp) produced by treatment planning for head-and-neck patients treated with IMRT. This experiment adopted 0.1mm ultrathin thermoluminescent dosimeters(ultrathin TLD) and ISP Gafchromic EBT film as the measured tool to establish the characteristics. Superficial doses at hypothetical neck lymphatic regions were measured with IMRT head/neck phantom and anthropomorphic Rando phantom. Seven-field IMRT plans with a prescribed dose of 180 cGy were designed on two different geometric phantoms using an Eclipse treatment planning system and used to measure the surface dose and the distances between isodose curve of Dp. Both mean surface measured doses for simulated phantoms with tumor 3mm shrinkage were within 5%. Both mean surface measured doses with percentage of prescribed dose of 180 cGy were 56.94 %, 56.23 %, 52.69 %, 55.25 %, 57.70 %, 54.46 %, 52.63 % and 53.74 % on right side neck; 63.73 %, 59.68 %, 55.37 %, 62.13 %, 61.87 %, 57.71 %, 62.67 % and 60.91 % on left side neck of IMRT head/neck phantom, respectively. Both mean surface measured doses with percentage of prescribed dose of 180 cGy were 56.62 %, 59.11 %, 66.53 %, 61.87 %, 62.83 % and 67.02 % on right side neck; 64.48 %, 63.25 %, 64.74 %, 67.31 %, 68.56 %和66.54 % on left side neck of Rando® phantom, respectively. From the experimental results, the surface doses and the isodose curve of Dp were related with the distances between the surface doses and the isodose curve of Dp. Surface dose increases with decreasing the distances between the surface dose and the isodose curve of Dp. The distances between the surface doses and the isodose curve of Dp were used to evaluate surface dose. Actually more experiments and statistics are needed to analyze and gather for surface dose. |
