中文摘要 | 近年來許多臨床研究給予冠狀動脈電腦斷層(CTA)精確度有極高評價,但是,CTA主要仍是解剖性的影像檢查,在某些狀況下會產生和臨床症狀出現不一致的結果;核醫心肌灌注造影(MPI)能精確診斷心肌缺血程度、預測冠狀動脈疾病(CAD)的預後並作為是否選擇侵襲性治療的參考。本研究目的是探討CTA與MPI之關聯性,藉以提供臨床醫師需進一步評估時之參考,總共收集40位受檢者分別執行64-多切面CTA以及MPI兩種檢查。經本研究分析,當CTA在冠狀動脈狹窄程度≥70%預測MPI心肌缺血的陽性預測值是87.5%,CTA在冠狀動脈狹窄程度50%~70%預測MPI心肌缺血的陽性預測值是60%。由本研究結果,CTA在冠狀動脈狹窄程度≥70%時與相應的MPI顯示心肌缺血之間的一致性較高,而CTA在冠狀動脈狹窄程度在50%~70%的患者中兩者的結果差異較大。 |
英文摘要 | Coronary computed tomography angiogram (CTA) had much high evaluations because of its accuracy. Some clinical conditions had different results with diagnosis. The major medical centers in Taiwan have health testing services to provide the myocardial and heart diseases examinations. The patients were decided whether to perform further invasive treatment (coronary artery stenosis≥85%) after checked by the clinician. MPI has assessed an optimal, non-invasive method to evaluate coronary artery diseases (CAD) for clinical relations of the diagnosis by the doctors. Estimating coronary artery diseases of 40 patients were estimated by 64 multi-slice computed tomography and myocardial perfusion imaging. The diagnosis of the coronary artery diseases was that CTA in coronary artery stenosis ≥ 70% to predict MPI positive predictive value of myocardial ischemia was 87.5%. CTA coronary artery stenosis in 50% to 70% of the predicted MPI positive predictive value of myocardial ischemia was 60%. By the results of this study, CTA in the coronary artery stenosis ≥ 70% with the corresponding MPI shows a high consistency between myocardial ischemia. CTA in coronary artery stenosis of 50% to 70% was compared to have the differences with results. The main purpose of this study is to analyze medical images and have diagnostic evaluations in the coronary artery diseases. |
