中文摘要 | 由於人體的顏面神經的走向相當的複雜,需經過中耳、內耳道及乳突部等複雜的解剖位置,所以想要在電腦斷層橫切面及冠狀切面影像中,將顏面神經同時完整呈現是不可能的。因此本研究就是要利用曲線平面重組的模式,將整體顏面神經串連起來並把周遭的解剖位置表現於同一影像平面上,如此就能清楚分辨腫瘤的出處與侵犯之範圍,好初步篩檢顏面神經的病變,讓有病變之病患可以做進一步的檢查,此種將顏面神經於同一影像完整呈現的技術,也是目前磁振造影檢查無法造影出的技術。本研究為期二十四個月,連續並不中斷共收集68位疑似顏面神經病變之病患,且都接受顳骨部位高解析度電腦斷層攝影掃描檢查,其檢查結果最後由放射科醫師及耳鼻喉專科醫師一同評估,共發現有10例病患(男性7位,女性3位),於曲線平面重組之影像中,可看到在顏面神經所經之處,有長出疑似之腫瘤。最後這10位患者皆再進一步接受核磁共振檢查,始確定這些病患患有顏面神經瘤。顏面神經瘤為一種罕見且多為良性之神經腫瘤,其病之臨床症狀並不特殊,而且在攝影診斷上仍然有部分不易克服之處,所以造就此病之誤診率依舊居高不下。因此如果能先藉由多排偵測器電腦斷層掃描儀,將掃描後之影像,以曲線平面重組模式表現出整條顏面神經,來判別有無顏面神經腫瘤,進而將非顏面神經瘤之病患排除在外,如此就能只針對顏面神經瘤患者,做更進精密的檢查。 |
英文摘要 | As techniques improve, many hospitals have installed multi-detectors computed tomography (CT). The application and research of facial nerve based on CT are obscured and hard to identify the complex structure. The courses of facial nerve are rather complicated; it passes many anatomic structures such as middle ear, internal acoustic canal and mastoid portions. It is unlikely to show the whole course based on conventional transverse and coronal sections of CT. The present study used curved planar reformation (CPR) to reconstruct the whole facial nerve course and surrounding anatomic structures in an imaging plane for check-up and screening of the facial nerve pathology. Multi-detector high-resolution CT was applied to 68 patients with suspicious facial nerve palsy for two years in this study. A neuroradiologist and an otolologist jointly assessed the results. We found 10 cases of facial nerve pathology (7 male patients and 3 female patients). There are facial nerve tumors along the courses in the facial nerve in the curved planar reformation. The 10 cases underwent further MRI examinations to reveal facial nerve neuroma. Facial nerve neuroma is a rare disease and mostly is benign. Its clinical symptoms are not specific. The imaging diagnosis by conventional CT is too difficult to be correctly diagnosed. If we could reconstruct the imaging data dedicatedly to show the whole courses of facial nerve by multi-detector CT, we can discriminate and screen the patients of facial nerve tumor. The clinicians could use this modern technology to increase the odds of correct diagnosis for facial nerve pathology. |
