| 中文摘要 | 本研究目的為評估數位元乳房X光攝影特有的曝露技術-以自動方式選取曝露控制(Automatic exposure control, AEC)偵測器位置,相較於手動選取AEC偵測器位置比較二者輻射劑量的差異與臨床使用的優缺點。方法為採用自動和手動選取AEC偵測器位置,各取樣1220張乳房影像,以廣義估計模式檢定此二不同選取方式及其平均乳腺輻射劑量的差異,並分析乳腺緻密度與壓迫厚度對輻射劑量的影響。結果中在自動選取AEC偵測器位置的平均乳腺輻射劑量為2.34±0.75mGy,手動選取者則為2.26±0.74mGy (P值=0.02)。當乳房內有局部組織不對稱、腫塊等問題,自動選取方式的AEC偵測器位置會落在腫塊處,導致受檢者承受過高的輻射劑量。結論,自動和手動選取AEC偵測器位置均能達到臨床醫師的影像標準,雖然自動較手動選取有較高一點的輻射劑量但臨床上是認同採用的;除了特定個案,建議應該採用手動選取AEC偵測器位置,來降低輻射劑量。 |
| 英文摘要 | Mammography has been paid a lot of attention during past 10 years, Several medical institutions kept studying on improving mammographic technologies and quality control. The diagnostic acceptability in the digital mammography is superior to that in the film-screen mammography for the early detection of breast lesion. The purpose of this study is to evaluate the radiation doses using the novel function for automatic exposure control (AEC), ie. automatic selection of sensor positions, and to compare radiation doses from automatic selections of sensor positions with those from manual selections. The limitations of using the automatic selection function were also discussed. A total of 2440 clinical images from manually and automatically selecting sensor positions were collected. Technical factors, ie. tube voltages, current-time products, compressed thicknesses, and average glandular doses, were recorded. The level of breast density was determined by an experienced technologist, Statistical analysis was also performed. Radiation doses from automatically-selected AEC sensor position were slightly higher than those from manually-selected. Relationship between the radiation dose and the breast density or the compressed thickness is not significant. However, excessive doses for patients were caused by the AEC sensors automatically located at the position of the dense tissue, such as focal lesions, masses, or popcorn calcifications, in the mammogram. The benefit of automatically-selected AEC sensor position is to avoid the incorrect choice of sensor positions by technologists. Furthermore, the rate of repetitive examinations could be reduced and the examining throughput could be improved. However, patients with dense breast lesions probably received higher radiation doses. Thus, changing automatic function to manual function is needed to avoid excessive dose for the following views. |
