中文摘要 | 背景:下巴疤痕攣縮外翻會導致嘴唇無法避合、流口水,影響外觀造成病人心理及社交障礙,適當的疤痕矯正與皮瓣重建可以提供有效的治療。目的及目標:我們成功的利用鼻唇皮瓣來重建一位下巴疤痕攣縮外翻的病患,得到良好的功能與外觀。材料及方法:一名79歲男子過去因為顏面創傷而導致下巴疤痕攣縮進而引發下唇外翻,有嘴唇無法避合及流口水之症狀,一開始我們選擇了全層皮膚移植手術,但病患經三個月追蹤後攣縮復發症狀無法緩解,我們最後將疤痕完全鬆解並且設計了左側鼻唇動脈的鼻唇島狀皮瓣來重建下巴疤痕鬆解後的缺損。結果:皮瓣手術成功,經過術後六個月的追蹤,病患已沒有下巴疤痕攣縮導致下唇外翻及流口水的症狀,外觀與心理社交獲得大幅改善。結論:根據此病例之治療經驗,鼻唇皮瓣對於下巴缺損之重建是一個簡單又快速的好方法。 |
英文摘要 | Background: Traumatic scar contracture on the lip–chin junction may induce lip eversion, drooling and psychosocial withdrawal. Adequate scar release and flap reconstruction were effective treatment. Aim and objectives: We reported on a successful reconstruction of lip eversion caused by scar contracture by using a nasal labial flap. Materials and Methods: A 79-year-old man developed lip eversion due to scar contracture after a trauma. The consequent severe drooling and facial disfigurement induced secondary psychosocial trauma. Initially, we simply released the scar tissue and resurfaced the wound with a full-thickness skin graft (FTSG), but it ended up re-contracture and lip eversion again. Then, we excised scar completely and resurfaced the wound by transplantation of a nasolabial island flap. Results: The flap survived well. There was no more scar contracture and no more lip eversion within a 6-month follow-up. The patient was mentally stable for good appearance without drooling. Conclusion: Nasolabial island flap is a reliable and easily available arterial flap for reconstruction of facial wound, especially in this case as it successfully corrects lip eversion after a complete scar contracture release. |
