中文摘要 | 胃腸道基質瘤 (gastrointestinal stromal tumor; GIST) 是罕見腸胃道惡性腫瘤之一, 台灣發生率約每百萬人10~20人,好發於55-65歲中高年齡者,目前約一千七百多位病 友。不易早期發現,通常是透過其他手術發現。對原發性沒有轉移的病人而言,手術 治療仍是最主要的治療方式。而對於復發或轉移病人的治療、或預防復發,可以使用 標靶藥來治療。目前治療藥品以酪氨酸激酶抑制劑為主,包括有 imatinib、sunitinib 和 regorafenib。Imatinib 為第一線用藥,治療劑量是每天投與400-600 mg。對於以 imatinib 治療期間出現疾病惡化或對該藥出現不能耐受副作用時,sunitinib 是二線替 代藥品,治療劑量是每天一次投與50 mg,按照治療4週接著停藥2週的時間表給藥。 Regorafenib 則是屬於第三線藥品,主要用於接受過 imitinib 和 sunitinib 治療後但無法 手術切除的局部晚期或轉移的 GIST,建議劑量是一個服藥週期為28天,每週期的前 21天,每天口服160 mg。因為標靶藥的使用,大幅提高此疾病病人的存活時間。 |
英文摘要 | Gastrointestinal stromal tumors (GIST) is one of the most common mesenchymal tumors in gastrointestinal tract. They can occur at any age, the prevalent age wound be 55-65 years old, and there are more than about seventeen hundred patients in Taiwan. It is hard to be early diagnosed, and is usually found during other surgery. Surgical resection is the "gold standard" for therapy of GIST. Recently, target therapy with inhibitors of tyrosine kinase receptors (imatinib, sunitinib,and regorafenib) has been introduced for the management of advanced and metastatic tumors. The first-line treatment is imatinib. The dose for adult is 400-600 mg once daily. For patients with metastatic GISTs fail to the treatment with imatinib, sunitinib could be given as the second-line treatment. The dose for adult is 50 mg once daily for 4 weeks, then followed by 2 weeks off. The third-line treatment of drug is regorafenib. The dose for adult is 160 mg daily for the first 21 days of each 28-day cycle. Early diagnosis and treatment would save many lives of patients with GIST-related emergencies. We strongly advocate that all patients with a GIST should be carefully and regularly followed up for longterm period. |
