| 中文摘要 | 上顎門齒管和正中門齒牙根相當接近,在植牙時應仔細評估兩者問三度空間之關係。本研究使用錐狀射束電腦斷層掃描量測,分析上顯門齒管及其前側齒槽骨之解剖特徵,以作量測,記錄數值包含門齒管鼻部開口、中段及腭部開口之直徑與其前側齒槽骨厚度、牙骨質牙釉質交界到門齒管腭部開口垂直距離,並比較男女及有無上顎正中門齒之患者各測量值之差異。無上顎正中門牙缺牙之患者,牙骨質牙釉質交界到門齒管腭部開口垂直距離為10.44±2.80mm,門齒管直徑平均為頰腭向:3.73±1.35mm,近遠心向:4.29±1.31mm;門齒管前骨厚度:6.89±1.69mm。上顎正中門齒缺牙組和無缺牙組比較,門齒管前骨厚度顯著較薄(P<0.0001)。男性門齒管高位之前側骨厚度較女性厚(7.67/6.15mm, P=0.002),而門齒管中段近遠心向及頰腭向直徑皆較女性窄(3.59/4.47mm; 3.08/3.80mm, P=0.002及0.03)。綜合以上數值顯示上顎正中門牙在植牙時,因解剖位置接近,確有可能侵犯門齒管,特別在靠近植牙的根尖部。 |
| 英文摘要 | A close anatomic relationship between the roots of the central maxillary incisors and the incisive canal is crucial in implant treatment in the premaxilla area, The aim of the present study is to analyze the anatomic characteristics of incisive canal on cone beam computed tomography (CBCT) and to evaluate the relationship between bone anterior to the canal and dental implant surgery, A total of 60 Taiwanese patients were included in the present study, The following measurements were taken from CBCT sections for analysis: 1) mesial-distal/buccal-palatal diameter of the incisive canal; 2) thickness of the bone anterior to the canal; 3) distance between cementum-enamel junction and palatal opening of the canal. All measurements are compared between males and females, between patients with missing and presence of central incisors, Results showed dimensions of incisive canal in patients without missing any maxillary incisors: buccal-palatal diameter: 3.73±1.35mm; mesial-distal diameter: 4.29±1.31mm; anterior bone thickness: 6.89±1.69mm; distance from CEJ to palatal opening of incisive canal: 10.44±2.80mm. Bone thickness anterior to incisive canal of patients with missing maxillary central incisor(s) is significantly less than non-missing group (P<0.05), Values of males are greater than females in bone thickness anterior to incisive canal (7.67mm and 6.15mm, P=0.002), but less in mesial-distal and buccal-palatal incisive canal diameter in the middle level (3.59/4.47mm; 3.08/3.80mm, P=0.002 and 0.03), In summary, the close anatomic relationship between incisive canal and maxillary central incisors, shows that implantation in maxillary central incisor edentulous area may damage incisive canal, especially near the apex of implant. |
