中文摘要 | 背景:肥胖已成為全球首要的健康問題,而國人十大死亡原因中,有8項與肥胖有關。研究發現,基因或生活習慣是造成肥胖的重要原因,但是「環境」也是不容忽視的關鍵影響因素,因此唯有同時透過飲食及運動的教育介入加上環境改變,才能克服多數人的肥胖問題。目的:(1)發展社區本位的肥胖控制介入計畫、(2)了解教育介入對肥胖控制的成效。方法:本研究採行動研究設計,介入項目包含致胖環境改變及飲食與運動的教育介入。在致胖環境改善的介入措施包括:建置本鄉鎮的運動地圖、輔導鎮內14家便利商店及5家自助餐廳供應健康餐;推動職場、社區、學校提供健康餐飲與增加身體活動空間、組織建立10個運動團體、透過保健志工的訓練建立支持性網絡; 飲食與運動的教育介入部份則是透過健康促進活動宣導包含認識健康飲食製作與選擇、規律運動的技巧、及教導民眾計算食物熱量、自我監督體重與生活型態。本研究收集苗栗縣某鎮共10個里,603名20-64歲以上居民,完整之參與介入活動前後的體重、身體質量指數(BMI),及腰圍資料。以配對t檢定進行單組前後測,探討介入成效。結果:介入前民眾體重範圍51至105公斤,平均65.11(10.92)公斤,介入後民眾體重範圍47至90公斤,平均62.98(10.54)公斤,居民平均減重2.12(1.24)公斤,配對t檢定顯示達統計上顯著差異(p<0.001);介入前民眾BMI範圍20.2至43.3kg/m2,平均25.49(3.54) kg/m2,介入後民眾BMI範圍19.5至41,平均24.46(3.45) kg/m2,介入後減少1.03(2.98) kg/m2,配對t檢定顯示達統計上顯著差異(p<0.001)。介入前腰圍平均84.88(10.44)公分,介入後腰圍平均83.27(9.99)公分,共減1.60(2.26)公分,配對t檢定顯示達統計上顯著差異(p<0.001)。顯示肥胖防治活動可有效減輕民眾體重、減少BMI與腰圍。此外民眾對於參與各類活動反應熱烈,期待下次的聚會活動。結論/實務應用:配合環境改變及以飲食與運動的教育介入確實能有效控制肥胖,建議未來特別加強抗拒外食誘惑的決心、運動技巧的學習與增強支持性網絡氛圍。 |
英文摘要 | Background: Obesity has become the world's premier health problems. The study found that genetic or lifestyle is an important reason for causing obesity, but the key factors "environment" cannot be ignored. Purposes: (1) the development of community-based intervention programs to control obesity; (2) to understand the effectiveness of educational interventions on obesity control. Methods: This study used an action research design; intervention project contains fat-induced environmental changes and diet and exercise education intervention. In a letter to the fat of environmental improvement interventions include: to build dynamic map of the town; Counseling 14 convenience stores and 5 buffet restaurants to serves healthy meals, promoting workplace, communities; schools provide healthy food and increasing physical activity empty Inter-organizational groups to build 10 sports, build support networks through health volunteer training. Diet and exercise educational intervention and partly through advocacy for health promotion activities included: knowledge production and choose a healthy diet, regular movement skills, and teach people calorie counting, self-monitoring body weight and lifestyle. We collected data from Miaoli County in a town, participation of 603 residents over the age of 20-64 year old, weight events before and after the intervention in, Body mass index (BMI), waist circumference. Paired t test were measured before and after a single group to explore the effectiveness of intervention. Results: The intervention before people weight range 51-105kg, with an average 65.11 (10.92)kg, the people involved in the weight range 47-90 kg after an average of 62.98 (10.54) kg, the average weight loss residents 2.12 (1.24) kg, paired t test revealed the difference was statistically significant (p<0.001); intervene before the BMI range of 20.2 to people 43.3kg/m^2, an average of 25.49 (3.54)kg/m^2, the people involved in the range of 19.5 to 41 BMI, mean 24.46 (3.45)kg/m^2, After the reduction of 1.03 (2.98) intervention kg/m^2, pairing a significant difference (p <0.001) reached statistical t-test display. Involved in the front waist average 84.88 (10.44) cm, waist average 83.27 after intervention (9.99) cm, a total reduction of 1.60 (2.26) cm, paired t test revealed significant differences (p<0.001) statistically. Show obesity prevention activities people can effectively reduce weight, reduce BMI and waist circumference. Conclusions/Implications for Practice: With environmental changes in diet and exercise and education intervention is indeed effective in controlling obesity. To recommend particularly strengthened to resist eating out, motor skills and enhance learning supportive network in the future. |
