中文摘要 | 目的:外科手術前校正患者的血小板功能,有時會引發嚴重的後遺症,如輸血引起的感染、心肌梗塞、或死亡。所以在口腔手術前不必要的血小板功能校正必須要避免。本文的目的在於找出血小板凝集時間過長患者未經血小板校正,手術拔牙及非手術性拔牙後出血機率,以及手術拔牙相對於非手術性拔牙後出血的相對風險。步驟及方法:這是一個回溯性觀察研究。主要觀察手術拔牙及非手術性拔牙患者拔牙後出血的狀況。所有患者都以血小板功能測試(platelet function analyzer-100)證實為血小板凝集時間過長。所有拔牙後出血的狀況都納入分析。結果:總共的位患者,13位接受手術性拔牙,27位接受非手術性拔牙。拔牙後出血的機率為2/40(0.05)。手術拔牙相對於非手術性拔牙後出血的相對風險為2.08(p = 0.59, power = 0.08)。結論:血小板功能測試時間過長的患者拔牙後出血的機率很低。但是手術拔牙相對於非手術性拔牙後出血的相對風險因為樣本數過小而無法提出可靠的結論。 |
英文摘要 | Purposes: Correction of platelet function before surgical procedures may sometimes cause serious problems, such as blood transfusion related infection, myocardial infarct, or even death. Therefore, ideally, unnecessary correction of platelet dysfunction should be avoided before oral surgery. The purposes of this study are to identify the incidence and relative risk of bleeding after dental extraction with either surgical or non-surgical approach in patients with abnormal platelet function but without platelet function correction. Methods: A retrospective cohort analysis of bleeding after dental extraction in patients with either surgical or non-surgical methods. All patients in both cohorts had coagulopathy as studied with platelet function analyzer-100 (PFA-100). Bleeding episodes were recorded and analyzed. Results: A total of 40 patients with 13 surgical and 27 non-surgical dental extractions were included in this study. The overall bleeding incidence is 2/40(0.05). The relative risk for bleeding in surgical extraction group versus non-surgical extraction is 2.08 (p=0.59, power = 0.08). Conclusions. The result reveals lower incidence of bleeding in dental extraction patients with prolonged PFA-100 test. Conclusion regarding to the relative risk of bleeding between surgical and non-surgical dental extractions cannot be made because of small sample size. |
