| 中文摘要 | 背景:電擊傷可以區分為高伏特傷害、低伏特傷害、閃電傷害以及電弧傷害。電流通過身體時會造成電化學反應而產生毒性產物造成組織裡氧氣濃度及PH值的改變。這些變化導致了組織的傷害。大多數的電擊傷發生在工作場合,且傷者以年輕人為主,因此電擊傷造成的併發症常造成社會及經濟的問題。目的及目標:本研究的目的在回顧台北榮總過去15年治療電擊傷的經驗。材料及方法:從1997年9月至2012年4月,總共收集59位病人的資料,並針對臨床表現及治療加以分析。結果:59位病人皆為男性,平均年齡為37.7歲。電流的入口最常見於手部,但電流出口位置就相當多變。高伏特電擊傷的病人有較高比例之意識喪失、較高之肌酸激酶數值、較長之燙傷中心停留時間及較長之住院時間,以及較多次之手術治療。若病患合併一開始有意識喪失之情形,其有較高的比例合併心臟方面的異常。此外,所有的緊急筋膜切開術、高處墜落傷害以及截肢手術皆與高伏特電擊傷有關。其中死亡率為1.7%,而34%的病人有功能性方面的後遺症。結 論:由於電流在人體流經的路徑之不可預測性,而導致臨床表現的多樣性。醫師必須了解電擊傷各種 可能的表現,以利於及早發現潛藏之傷害,及早治療,以降低後遺症的比例。 |
| 英文摘要 | Background:Electrical injuries could be divided into high-voltage injuries, low-voltage electrical injuries, lightning strikes, and electric arc injuries. When the electrical current passes through the body, it causes electrochemical reactions as exothermic effects and generates toxic chemical by-products that alter tissue oxygen and pH levels. These reactions lead to tissue damage. Most of electrical injuries are noted in workers injured on their jobs. The loss of these employed young people from their jobs and the consequent high morbidity rate lead to a significant socioeconomic issue.Aim and Objectives:The purpose of this study is to report our experience of treating electrical injuries in our institution in the past 15 years.Materials and Methods:From September, 1997 to April, 2012, fifty-nine patients were included in our study. The clinical manifestations and management were reviewed.Results:All 59 patients were male. The mean age of all the patients was 37.7 years. The hand was the most common site of the inlet of the electric current, but the sites of the outlets varied. In the high-voltage injury group, the patients had a higher rate of the initial loss of consciousness, higher significant CK (creatine kinase) levels, longer burn center and hospital stays, and more surgical intervention. If the patients had initial loss of consciousness, they had a significant higher rate of cardiac abnormality compared with those without initial loss of consciousness. Moreover, the high-voltage injury group corresponded to all emergent fasciotomy and had accidents of falling down and limb resections. The overall mortality rate was 1.7%, and 34% of patients had functional complications.Conclusion:Due to the unpredictable electrical current pathway in a human body leading to a broad spectrum of clinical manifestations of electrical injuries, physicians have to comprehend all the possibilities of the presentations of electrical injuries for early detection and early treatment to prevent further morbidity. |
