中文摘要 | 研究背景:心臟病患於手術創傷後大多有呼吸以及肌肉骨骼系統限制,除接受傳統心臟復健計畫之外,還未有融合強調呼吸調節與核心肌群控制的皮拉提斯方法應用於此族群的研究。目的:探討融合皮拉提斯訓練於第二期心臟復健計畫之呼吸療效。方法:2011年2月至12月間於振興醫院接受心臟手術、第一期心臟復健,且能完成運動測試之病患。23名接受融合皮拉提斯訓練之第二期心臟復健(Pilates-based cardiac rehabilitation, PCR);未參與第二期心臟復健、僅接受一般醫療照護者14名為控制組;並選取2004至2005年間接受傳統心臟復健(Conventional cardiac rehabilitation, CCR)、運動監測頻率配對之18名病患。兩心臟復健組以運動測試結果訂定個別運動量,為期8-12週、每週1-3次、每次40-50分鐘的中等強度有氧訓練,以及50分鐘的團體運動課,PCR組融入皮拉提斯方法,CCR組則無。三個月後進行第二次運動測試,利用重複量測共變異數分析來檢定三組在靜態與動態呼吸參數的改變。結果:調控心臟術式與術後時間後,靜態與動態呼吸量沒有顯著差異,三組之運動尖峰死腔/潮氣容積比、攝氧量、二氧化碳排出量、以及二氧化碳換氣當量斜率(VE/VCO2 slope)都有顯著進步,且僅PCR組之VE/VCO2 slope有時間與組別之交互作用(p=0.029)。結論:應用皮拉提斯方法輔助心臟復健計畫,是一對呼吸有潛在幫助可以選擇納入的元素。 |
英文摘要 | Background: Sternotomy of the cardiac surgery impaired the respiratory function and chest wall movement. Breathing control and core muscle training are essential components of Pilates exercise, which may match the requirement of the population. Purpose: We designed a Pilatesbased cardiac rehabilitation (PCR) program to examine the effect on respiratory function in patients underwent cardiac surgery. Methods: We recruited the patients who underwent cardiac surgery, received phase 1 CR, and completed the exercise test in a heart center of Cheng Hsin General Hospital. Patients who attended phase 2 CR program were classified to PCR group (Feb. to Dec. in 2011) and conventional CR (CCR) group, without Pilates exercise (Mar. 2004 to Jan. 2005), otherwise to control (C) group. Patients in the exercise groups both participated a 40-50 minutes individual aerobic training and 50-minute group exercise 1-3 times a week. Pilates method was applied in PCR group, but not in CCR group. Static and dynamic respiratory function were repeatedly assessed after 3 months. Repeated measure analysis of covariance and Bonfferoni test were used to examine the effect of the intervention. Results: There were no significant change of the static and dynamic respiratory volume, but significant change of dead space, oxygen consumption, volume of carbon dioxide, and the minute ventilation - carbon dioxide production relationship (VE/VCO2 slope) in 3 groups. However, only VE/VCO2 slope had group and time interaction in PCR group (p=0.029). Conclusions: Pilates method could be considered an innovative and potentially effective element to dynamic respiratory function in cardiac rehabilitation program. |
