| 中文摘要 | 背景及目的:在台灣,大約有30%的糖尿病患者血糖控制不佳,也就是糖化血色素的值大於9%。研究顯示,當糖化血色素的值下降1%時,患者心臟血管疾病或是心肌梗塞的風險便會因此下降12%-16%。方法:本研究蒐集35位糖化血色素大於9%之第二型糖尿病控制不良患者參與。採用單一族群前後對照比較法,採用糖尿病自我照顧量表,探討介入前後糖化血色素的差異以及評估患者自我照顧相關知識及照顧能力在介入前後的差異。結果:本研究樣本數為35位,女性居多為25人(71.43%),罹患糖尿病時間介於1-40年,其罹病平均時間為12.26年(±8.08年)。介入前糖化血色素平均值為10.08%(±1.01%),介入一年後,平均值為9.34%(±1.28%),以配對t檢定檢測,呈現糖尿病共同照護的積極介入使得糖化血色素有顯著地下降(p value < 0.01),糖尿病自我照護的認知與能力亦有顯著提升(p value < 0.01)。結論:本研究發現透過糖尿病共同照護的積極介入,使個案能夠遵從自我照顧技巧、規律運動、飲食及藥物控制,將可改善糖化血色素值,同時提升病人自我照護能力。 |
| 英文摘要 | Background and objectives: The serum glucose level of approximately 30% of patients with diabetes mellitus (DM) in Taiwan was poorly controlled, which means the value of glycosylated hemoglobin (HbA1C) is greater than 9%. Some studies have shown that the risk of cerebrovascular diseases and myocardial infarction will be reduced by 12%-16% if the HbA1C value decreases by 1%. Methods: A total of 35 type 2 DM patients with HbA1C greater than 9% participated in Diabetes Shared-Care Program (DSCP), a DM intervention program to provide frequent interactions between patients and healthcare providers, and multiple educational courses to patients. A structured questionnaire with a five-level Likert scale was used to evaluate the knowledge of DM self care. Paired t test was used to compare the before-DSCP with after-DSCP knowledge of DM self care by the questionnaire, and HbA1C. Result: Twenty-five (71.43%) of the participants were female. The time that the patient has DM ranged from one to 40 years, with an average of 12.26 years (± 8.08 years). Before the intervention of DSCP, the mean HbA1C was 10.08% (± 1.01%). After the intervention, a significant reduction of 0.74% in HbA1C was shown. According to the questionnaire, the after-DSCP knowledge of DM self care was 4.06 (± 0.44), which is significantly higher than the before-DSCP one (3.67 ± 0.51). Conclusion: This study found that, by the active intervention of Diabetes Shared-Care Program, type poorly controlled type 2 DM patients complied with self care skills, regular exercise, diet and drug control, and thus being able to improve HbA1C value, and also patient self-care abilities. |
