| 中文摘要 | 目的:記憶缺失是中風患者常見的認知問題之一,可能進一步影響其日常活動表現,但迄今探討兩者之相關性的研究則十分缺乏。故本研究的目的在於檢驗中風患者之各種記憶缺失與其日常活動功能之相關性。方法:共計15名中風個案參與本研究。以情境式記憶測驗及行為記憶測驗第三版之兩項子測驗評量個案的回溯性記憶、前瞻性記憶和後設記憶等功能,以功能性獨立量表和諾丁漢日常活動量表分別評估其基本和操作性的日常功能。結果:60%的受試者顯示有回溯性記憶缺失,但多數患者自覺記憶功能良好,有86.6%的患者無法使用具體的策略協助本身的記憶表現,顯示有明顯之後設記憶缺失。患者在與時間性質之前瞻性記憶亦有所損傷。相關分析顯示受試者的回溯性記憶和後設記憶主要會受到年齡、基本認知功能和中風嚴重程度的影響,並與其日常功能有顯著的相關。結論:本研究結果顯示中風患者的記憶功能確實不佳,且影響其日常活動表現。故建議臨床對中風患者的處置應重視各類記憶功能的評估和訓練,以提昇患者的日常功能。 |
| 英文摘要 | Objective: Little is know about the relation between memory impairments and functional performance in stroke. The purposes of the study were to examine memory impairments following stroke and to explore the impact of memory impairments on daily function. Methods: A total of 15 patients with stroke were recruited in this study. The Contextual Memory Test and two subtests of The Rivermead Behavioral Memory Test were used to examine different types of memory. The Functional Independence Measure and Nottingham Extended ADL Scale were used for evaluating ADL functions. Results: Sixty percent of participants demonstrated retrospective memory impairments. Most of participants revealed lack of self-awareness and 86.6% could not use of memory strategy, which indicating deficits in metamemory. All participants also showed poor time-based prospective memory. Retrospective memory and metamemory were associated with age, basic cognitive function and stroke severity. Significant correlations between impairments of retrospective memory and metamemory and ADL performances were also presented. Conclusion: The study revealed memory impairments in stroke with a negative impact on daily functions. Our findings suggest that comprehensive memory evaluation and intervention for stroke patients is needed to promote their daily functions. |
