| 英文摘要 | The present article describes a case involving a patient suffering from cerebral palsy and respiratory failure who underwent endotracheal tube placement during the nursing care period, April 2 to April 28, 2009. Based on an evaluation at four levels, namely physiology, mental state, societial, and innate intelligence, together with practical care, observation and the application of physical assessment techniques, which were linked to communication with family members in order to collect information, we concluded that the patient's problems fell into a number of major categories. These consisted of ineffective airway clearance, anxiety, caregiver role strain and some others. During nursing practice, the patient was assisted closely when expelling respiratory tract secretion in order to maintain his respiratory tract function. In addition, the physician and a respiratory therapist worked together to create a respiratory care program by which the patient could complete the training necessary to wean him from mechanical ventilation. By building up the treatment relationship between the nursing staff and the patient, by creating a familiar environment for the patient by decorating his environment, by accompanying the patient in a proper and timely manner, and by increasing the visiting time allowed for the patient's mother, the patient was positively supported. This resulted in a reduction in his sense of anxiety. A support group was established and patient information related to disease prevention, domestic care, and hospital discharge preparation was provided to the primary caretaker during hospital visits. As the result, the primary caretaker was able to understand how to care for the patient, how to reduce the patient's anxiety with respect to the disease and post-treatment care, and this will help with preventing and/or diminishing any reoccurrence of the disease. Overall, the above actions helped and will help the patient and his family members navigate through similar crises. |
