| 中文摘要 | 本篇報告是在探討一位全身嚴重燒傷個案,在復健期傷口日漸癒合時,面對出院準備之護理經驗。護理期間2009年8月3日到2009年11月13日,筆者以直接護理、參與式觀察及會談等方式收集資料,藉Gordon十一項健康型態進行評估,發現個案有疼痛、自我照顧能力缺失及照顧者角色緊張等護理問題。護理過程筆者以傾聽、主動關懷、接受及支持等技巧,與個案建立良好護病關係,協助個案及家屬在傷口復原後,執行疤痕復健照護,提供所需知識和相關後續照護安排,讓個案能接受及適應出院後的生活改變,增加調適及因應能力,來面對漫長的復健過程。 |
| 英文摘要 | The article explored the nursing experience of a patient with severe burns over his entire body until he was ready for discharge from the hospital as his wounds gradually healed during the rehabilitation period. Nursing took place from August, 3rd 2009 to November, 13th 2009, during which information were collected by means of direct nursing, participant observation and interviews. We made an evaluation using Gordon’s 11 functional health patterns, and found that the patient had nursing issues such as pain, lacked self-care skills, and caregiver tension. During the nursing process, we applied various skills, including listening, active caring, accepting and supporting, in order to establish a good nurse-patient relationship. We helped the patient and relatives to carry out scar and rehabilitation care after wound healing, simultaneously provide them with relevant knowledge as well as aiding with the follow-up care arrangements. The patient was able to accept and adapt to the changes in his life after discharge from the hospital and his adaptation and coping skills were improved; these adjustments gave him the tools necessary to face the long rehabilitation process. |
