| 中文摘要 | 對於無自主能力之末期腎病患者如智能障礙者,治療上以血液透析為首選。本個案報告為一位 34歲中度智能障礙女性,其主要照顧者-母親卻選擇以腹膜透析為其末期腎病的治療方式。筆者於 97年 12月16日至98年 1月 23日照顧期間,透過會談、觀察、理學檢查與臨床照護收集個案資料,將資料加以分析、歸納出三個主要健康問題:個案有疾病照護學習障礙、主要照顧者角色緊張和潛在危險性感染等問題。在護理過程中,筆者透過護病信賴關係的建立,讓主要照顧者相信個案具備部分基本的學習能力,也願意讓個案使用工作分析法之學習策略,成功地學會腹膜透析部分基本自我照顧能力及如何預防管路感染,進而減輕了主要照顧者的負擔。我們認為中度智障患者仍具備基本的學習能力參與簡單的自我照顧,於本文中提出照護智障患者的過程及經驗。 |
| 英文摘要 | For patients who are mentally retarded, hemodialysis is the treatment choice for the end stage of renal disease because they are unable to make self-responsible medical decisions in full self-attendance. We present a 34-year-old female with mental retardation receiving continuous ambulatory peritoneal dialysis as the choice for her end stage of renal disease. The medical decision was made by her mother, the primary caregiver. During the nursing care period from December 16th 2008 to January 23rd 2009, we collected data by interview, observation, physical assessment and nursing care. The learning disability for disease caring, the strain of the caregiver’s role and the risk for infections were considered to be the major health problems after analyzing the collected data. The nurse-patient relationship was established through the nursing process, and we supported the primary caregiver in believing that the patient had some basic learning ability. After the agreement of her mother, we used a「task analysis」learning strategy and the patient learnt basic self-care skills including prevention of catheter infection successfully. The strain and burden of the primary caregiver was reduced. In this study, we demonstrate that the nursing care process and experience has successful intervention for the patient with mental retardation and can foster her basic abilities of learning and self-care. |
