中文摘要 | 本文是探討一位57歲因脊髓損傷引發疼痛與焦慮、病患之護理經驗。護理期間自2011年8月20日至9月12日,運用Gordon十一項健康型態功能評估,以觀察、直接照護、氣切發聲器會談及病歷查詢等方式進行收集資料,確立主要之護理問題為1.呼吸道清除功能失效、2.慢性神經性疼痛、3 .焦慮,照護期間筆者針對護理問題與病患建立良好護病關係,提供個別性護理措施並教導有效性呼吸咳嗽技巧,運用拍痰機治療及醫囑輔助,協助痰液排除。另外,教導病患肌肉放鬆技巧及轉移注意力等多元化護理措施,並維持舒適臥位以減緩病患疼痛,安排復健與呼吸訓練計畫,經常陪伴在側、傾聽與鼓勵病患,進而改善焦慮感,引導病患能正向面對疾病所帶來的改變,使病情漸趨穩定減少併發症的發生,以獲得完善護理照護,有效提升生活品質。 |
英文摘要 | This article describes the nursing experience of caring for a 57-year-old patient with pain and anxiety resulting from spinal cord injury. Gordon's Functional Health Pattern Assessment was conducted during the caring period from August 20 to September 12, 2011. Data was collected via observation, direct care, communication with the assistance of an artificial larynx, and medical record review. The identified major nursing care problems consisted of ineffective airway clearance, chronic neuropathic pain, and anxiety. A good nurse-patient relationship was established for effective nursing intervention during the caring period. The patient was provided with individualized care and taught to perform effective breathing and coughing techniques. Sputum removal was facilitated by the use of a mechanical percussor and supportive medical treatment. In addition, we also teached the patient with diversified nursing interventions, such as muscle relaxation techniques, distraction and maintaining a comfortable lying position, to reduce pain. The patient was also assisted to arrange rehabilitation and respiratory training programs. Frequent accompaniment, listening and encouragement reduced the level of anxiety. The patient was also guided to develop a positive attitude toward the changes caused by the illness. With the comprehensive nursing care, his illness gradually stabilized with reduced incidence of complications and subsequently quality of life was improved. |
