中文摘要 | 組合徵候群(combination syndrome)的臨床症狀通常出現於上顎牙齒全部缺失且下顎僅前牙保存下來的局部缺牙患者口腔中,此症狀的表徵包括上顎前區無牙嵴嚴重吸收合併其它缺牙區域的擴大生長與萎縮變化等,牙科醫師面臨此困難處境將需透過全盤的考量以擬定最適切的治療計畫。傳統的處理方式為上顎全口義齒搭配下顎活動式局部義齒,但此種處置方式並非一致適用且患者會因骨頭持續性吸收而有需重新製作義齒的困擾。牙科植體贋復提供有別於傳統處置的骨頭錨定技術,藉由植體的有效應用來達到改善固持與支持式贋復體的固位性與穩定性。本篇文章提出可行性的植體處置模式,實際運用於上顎全口無牙面對下顎雙側遠端局部缺牙的臨床病例。 |
英文摘要 | Combination syndrome is a dental condition that is commonly seen in patients with a fully edentulous maxilla and partially edentulous mandible with preserved anterior teeth. This syndrome consists of severe anterior maxillary resorption combined with hypertrophic and atrophic changes in different quadrants of maxilla and mandible. This condition can present a challenging situation to the clinician who needs to assemble evidence from all aspects of dentistry to formulate an appropriate treatment plan. Conventional treatment with full upper and partial lower dentures is not always adequate or satisfying for patients and it often requires multiple remakes due to continuing bone resorption. Dental implant rehabilitation changes conventional treatment with bone-anchoring techniques to provide improved retention and stability for implant-retained and supported prostheses. This article presents practicable implant treatments in patients with completely edentulous maxilla opposing bilateral partially edentulous mandible. |
