英文摘要 | OBJECTIVE: In recent years, investigations surrounding idiopathic sudden sensorineural hearing loss (lSSHL) have focused on identifying its prognostic factors. Studies have shown that patients with favorable prognostic factors have better outcome with fewer interventions. Many have been identified, but blood biochemistry tests have not been systemically studied in this regard. The purpose of our study was to evaluate the factors that impact the outcome of ISSHL, with emphasis on blood biochemistry tests.METHODS: A retrospective medical chart review of patients who were diagnosed with ISSHL and hospitalized in the Department of Otolaryngology, Changhua Show Chwan Hospital from January 2005 to September 2010 was performed. A total of 229 patients were enrolled. Demographic characteristics (gender, age, left ear), time of onset (season of onset), major symptoms (vertigo, dizziness in the ears), time of delay to treat, hearing test results (initial level of hearing loss and pure tone audiogram in newly diagnosed type) and blood biochemical parameters were collected. The data was analyzed with Chi-square test and analysis of variance (ANOVA) to determine statistical significance.RESULTS: Younger age, lesser degree of hearing loss at initial diagnosis, pure tone audiometry picture showing type I (sloping up) at initial diagnosis, lower postprandial glucose (PC sugar), lower levels of low density lipoprotein (low density lipoprotein, LDL), total cholesterol (total cholesterol, TC), high density lipoprotein (high density lipoprotein, HDL), and erythrocyte sedimentation rate (Erythrocyte sedimentation rate, ESR) were statistically significant for favorable outcome of disease. (p < 0.05).CONCLUSION: This study confirmed previous reports that age, degree of hearing loss in newly diagnosed and newly diagnosed type pure tone audiogram have prognostic value for ISSHL. Additionally, biochemistry tests including PC Sugar, LDL, TC, HDL, and ESR also showed predictive value to the outcome of the disease. This may be important for clinicians in explaining the disease progression to patients, and also may further influence the choice and duration of treatment modalities. |
