中文摘要 | 本文旨在呈現一位38歲青年期口腔癌患者,住院期間無家人照顧和支持,且在面臨手術後生理和心理之不適時,出現了情緒低落、少語、無望感等負向情緒,經由筆者直接照護、運用身體評估、會談和觀察,並藉羅氏適應模式評估個案,收集主、客觀資料,歸納了主要健康問題有:無望感、呼吸道清除功能失效、口腔黏膜障礙、身體心像改變等。在此護理過程中以傾聽、主動關懷來接納患者心中感受,給予其支持鼓勵,協助接受治療及正視自己的需求;並教導深呼吸、咳嗽技巧和提供胸腔物理治療等措施,協助痰液的排出;加強口腔皮瓣及傷口照護使能有效地改善臭味及無感染產生;透過社工和牧師協助,建立起個案及家屬的支持系統,使其心靈上獲得平靜,進而重拾與家人互動關係。出院後更能主動加入病友關懷團體,和參加了區域性聯誼活動,不但身心靈不適獲得改善且成功戒掉抽菸、喝酒、嚼檳榔的習慣。筆者藉此護理經驗,提出與護理人員分享,以助能提升日後照護此類患者的參考。 |
英文摘要 | The case report is to describe nursing experiences of a 38-year-old patient with malignant oral tumor. The patient experienced the physical and psychological discomforts after surgery, with the lack of support and care from his families during hospitalization. He showed negative emotion such as depression, silence, and hopelessness. The authors provided direct care to the patient. Subjective and objective data was collected through physical examination, interview, and observation, and Roy's model was used to assess the patient. The patient's health problems were identified as hopelessness, airway clearance ineffectiveness, oral mucous membrane impaired, and body image disturbed. During the nursing process, the authors accepted the patient's feelings through listening, empathy and active caring and positive thinking, offered encouragement and support, and assisted him in receiving the treatment, and found his own needs. In addition, the authors taught the patient to implement deep breathing, cough, and chest physical therapy to help eliminating the sputum. The wound care was improved, no infection and no odor occurred. Under the help from the social worker and pastor, the patient re-built his family's support system, developed the interactions with his families, and gained peaceful mind and spirit. After discharge, the patient actively participated in the patient support group, and the social activities in the community, and had relieved his physical discomfort, quit smoking and drinking, and betel nut. This case report presents successful nursing experiences to care providers for patients with malignant oral tumor. |
