中文摘要 | 研究目的:本研究目的即為瞭解病人、主要照顧者及醫療相關人員之緩和療護品質及其差異性。研究方法:採橫斷式研究設計,在北部某安寧病房,以緩和醫療結果量表作為工具,同時收集安寧病人、主要照顧者、醫療人員等之三組資料,以獲得緩和療護品質的看法。研究結果:發現三類組群對於緩和醫療品質分數極為接近,且醫療品質良好,病人與醫療人員在提供資料及生命價值具有統計意義,醫療人員感受病人在提供資料及生命價值等項目的品質較病人本身評核差。主要照顧者與醫療人員在生命價值、家屬焦慮、及個人事務具統計意義(p<0.05),主要照顧者感受家屬焦慮及個人事務等項目的品質較醫療人員評核差,醫療人員感受病人的生命價值品質較主要照顧者評核差。結論:除了提供資料、生命價值、個人事務、家屬焦慮等四個項目外,醫療人員與家屬、主要照顧者在安寧療護品質上是有一致的看法,希望本研究可提供未來照顧之參考。 |
英文摘要 | Background: The evaluation of palliative care quality can understand care quality and it can be as an evidence for improving quality. In order to understand the internal consistency of medical professionals, patients and caregiver, health care system needs to be assessed appropriation of patient and caregiver. Previously, some studies focused on the medical quality care for patients’ and caregivers' views without medical professionals assessing the same situation, therefore, leading to biases in health care system. Objectives: The purpose of this study was to compare the palliative care quality among patients, caregivers, and medical professionals. Method: The cross-sectional research design was conducted. We used the palliative care outcome scale to assess the palliative care quality among the patients, the caregivers, and the medical professionals at the same time. The data were collected from a palliative care unit of the teaching hospital in Taiwan. Result: The results showed that the palliative care quality of patients and caregivers was in accordance with medical professionals and in ideal situation. The scores of the items,“life worthwhile and information”, had statistical significant between patients and medical professionals. The medical professionals perceived the items, “life worthwhile and information”, were poor than those of the patients. The scores of the items, “life worthwhile, family anxiety, and personal affairs”, had statistical significant between caregivers and medical professionals (p<0.05). The caregivers’ perceived the items,“family anxiety and personal affairs” were poor than those of medical professionals. The medical professionals’ perceived the item, “life worthwhile”, was poor than that of caregivers.Conclusion: Except the items, “information, life worthwhile, personal affairs, and family anxiety”, the scores of patients and caregivers were closed to those of the medical professionals. The results can be references to monitor continually the palliative care quality in the future. |
