中文摘要 | 背景與目的:包裹毛巾可以減緩熱敷包溫度下降速度,同時避免熱敷包直接與體表 接觸而燙傷皮膚。然而,目前對熱敷包毛巾包裹層數尚無定論。本研究藉由改變毛巾包 裹層數,探討毛巾包裹對於熱敷包產生體表溫度變化的影響。方法:20 位健康年輕成人 (男性5 位、女性15 位,平均年齡24.1±2.8 歲)參與本實驗。熱敷包先在72℃熱敷箱 中隔夜加熱,每位受試者的右側伸腕肌群肌腹必須分別接受包裹三種不同層數之乾毛巾 (4、6 與8 層)的熱敷包刺激。以藍芽無線生理回饋儀記錄熱敷前3 分鐘、熱敷20 分 鐘與熱敷後30 分鐘的體表溫度變化。每次實驗至少間隔24 小時。結果:不管毛巾包裹 層數,體表皆接可以達到40-45℃的治療溫度範圍內。與包裹6 層及8 層毛巾相比,包 裹4 層毛巾的熱敷包有顯著較高的體表最高溫度、較多的體表溫度增加量、在較短的時 間內達到最高溫度,以及在治療溫度持續的時間較久(p<0.05)。與包裹8 層毛巾相比, 包裹6 層毛巾的熱敷包有顯著較高的體表最高溫度、治療溫度持續的時間較久 (p<0.05);然而,在體表溫度增加量(95%信賴區間為-0.8℃至0.8℃,p=0.995)與達 最高溫度所需時間上(95%信賴區間為-2.8℃至0.1℃,p=0.062)則無顯著的差異。不管 毛巾包裹層數,熱敷結束後第5 分鐘與第30 分鐘的體表殘餘溫度皆顯著高於治療前的 體表溫度(平均差為0.7-1.4℃,p<0.05)。結論:熱敷包包裹毛巾層數愈少,體表在愈 短時間內可達到愈高的溫度、愈多的體表溫度增加量,以及獲得愈長的治療溫度時間。 |
英文摘要 | Background and purposes: Towel wrappings adapted in hot-pack application not only decrease heat loss from hot packs but also prevent from a risk of skin burning. However, the wrapped layers of towel have not well been established. The purpose of this study was to investigate the effect of wrapped layers of towel on the changes of skin temperature during hot-pack application. Methods: Twenty healthy young adults (5 men and 15 women, mean age 24.1±2.8 years participated in this study. Hot packs were heated to 72℃ by storing in hot water tank for overnight. Each subject performed three trials of hot pack heating on the skin surface overlying the right wrist extensor muscle bellies, wrapping with 4, 6, and 8 layers of towel respectively. At least 24 hours between trials was required. Bluetooth wireless physiological biofeedback was used to record the changes of skin temperature during pre-heating 3 minutes, heating 20 minutes and post-heating 30 minutes. Results: All the three trials can raise the skin temperatures to a therapeutic range of 40-45℃. Compared with 6 and 8 layers, application of 4 layers can significantly increase the maximum skin temperature, increase the amplitude of skin temperature rises, decrease the duration to reach the maximum skin temperature, and elongate the period to stay at the therapeutic temperature (p<0.05). Compared with 8 layers, application of 6 layers can significantly increase the maximum skin temperature and elongate the period to stay at the therapeutic temperature (p<0.05). However, the amplitude of skin temperature rises (95% CI, -0.8℃ to 0.8℃, p=0.995) and the duration to reach the maximum skin temperature (95% CI, -2.8℃ to 0.1℃, p=0.062) did not reach a significant level. Regardless of towel layers, the residual skin temperatures were significant higher (mean deviation 0.7-1.4℃, p<0.05) in the post-heating 5 and 30 minutes than in the pre-heating stage. Conclusion: The fewer towel layers adapted in wrapping a hot pack, the shorter duration required to reach the maximum skin temperature, the larger skin temperature rises, and the longer period to stay at the therapeutic temperature. |
