中文摘要 | 本文探討一位罹患風濕性心臟疾病之原住民女性,因未規則服用藥物及生活型態不 良,導致心臟功能嚴重受損,進而入院接受瓣膜置換術治療。照護期間自2009年11月23 至11月27日,筆者藉由主動關懷及醫護聯合討論會探討病程的變化及運用Gordon 11項健 康功能型態評估,確立其主要護理問題為:心輸出量減少、健康維護能力改變及潛在危 險性出血。整合個案醫療照護的迫切性措施為:維持血液動力學穩定,改善心輸出量; 協助個案落實自我照護能力並預防因原民活動引發的合併症;規則服用慢性病藥物及抗 凝血劑之重要性,說明藥物與生活習慣之相關性。鑒於本文結果,建議醫護人員應了解 原民文化與日常生活型態對疾病的影響,提供合適的護理措施並促進其健康。 |
英文摘要 | This paper presented a nursing experience of an aboriginal woman with rheumatic heart disease combined with aortic stenosis. The patient was admitted to the hospital to receive the valve replacement surgery due to severely impaired heart function, which was related to the tobacco, alcohol and betel nut abuse with irregular medication adherence. The duration of nursing care was from November 23 th to 27th of 2009. Through active care, exploration of the course of disease through joint discussions of health care professionals, and the application of Gordon’s 11 functional health patterns, major health issues were established: decreased cardiac output, impaired health maintenance and high risk for bleeding. Interventions for the patient based on her condition were as followed: maintaining the stabilization of hemodynamics to improve cardiac output; improve self-care ability through health education based on the patient’s cultural background; improving medication adherence such as anticoagulants and antihypertensive drugs by explaining the relevance between life style and medication. The results from this paper encourages health care providers to take patients’ cultural background and life styles into consideration and their potential impacts on the diseases, in turn providing appropriate nursing interventions to promote the patients’ health status. |
