中文摘要 | 目的:眼外直肌肌腹斷裂是外傷或眼球手術的嚴重併發症。即使接受肌肉縫合或轉位手術,眼球運動也會嚴重受限。吾人報告以懸吊式縫合成功修復兩例下直肌肌腹斷裂。方法:病例報告。結果:吾人以手術顯微鏡找出斷裂的下直肌肌肉近端。在縫合兩斷端後,吾人延伸兩縫線(懸吊)至下直肌肌腱附著處。在術後第一天,第一級眼位為正位。雖然初期眼球往下的功能受限,但輕微的頭部姿勢變化便可避免複視發生。結論:此技術可提供眼位立即性改善,藉自懸吊的縫線協助支撐損傷的肌肉,避免其拮抗肌產生攣縮。 |
英文摘要 | Purpose: Mid-belly rupture of a rectus extraocular muscle is a serious complication of trauma or ocular surgery. Severe restriction of eye movement was present even after muscle-to-muscle anastomosis or muscle transposition. We present a sling technique for mid-belly rupture of the inferior rectus muscle in two cases.Methods: Case reportResults: We utilized a surgical microscope to explore and retrieve the proximal end of ruptured inferior rectus muscles. After suture reunion of the muscle rupture, we augmented the repair by fixing the repaired segment to the original insertion of inferior rectus muscle with two sutures in a sling fashion. On the first postoperative day, orthophoria was found in primary gaze. Although function of the depressor was impaired at the initial postoperative period, recovery of infraduction was quite good. Both cases did not have diplopia in primary position and down gaze after six-month follow up.Conclusion: This technique provides an immediate improvement in ocular alignment and appears to support the avulsed muscle and helps to avoid contracture of the antagonist. |
