中文摘要 | 本文探討一位肝癌末期患者的個案合併骨與肺轉移,於住院期間的疾病照護經驗。護理期間自民國98年3月2日至98年4月9日止。照顧期間藉由觀察、會談以及身體評估等方式收集資料,運用安寧療護的生理、心理、社會與靈性的整體性評估,發現個案住院期間的主要健康問題有:一、慢性疼痛/與腹部脹痛、疾病進展有關;二、低效型呼吸型態/右肺肋膜積水壓迫有關;三、預期性哀傷/即將與家人分離以及面對死亡有關之心理問題。護理期間,個案對疾病的惡化出現憂鬱與哀傷的情緒反應,導致家人陷入悲傷中,為促使病患於疾病末期獲得舒適與尊嚴,並且藉由本文提供臨床護理人員照顧臨終病患須具備的知識與能力,筆者運用安寧療護的全人、全家、全程、全隊四全之照護理念,提供關懷、支持與陪伴,緩解個案身體不適之問題,並教導家屬臨終照護的技巧,讓個案接受疾病末期的事實,進而勇於面對死亡,以期達到身、心、靈三平安的目標。 |
英文摘要 | This case report is to describe a caring experience of a patient who suffered from hepatocellular carcinoma with bone and lung metastasis. The author took care of the patient from 23th March to 9th April 2009. The data was collected by means of observation, interview and physical assessment during the nursing process. The assessment tool of hospice care was employed to evaluate the patient's physiological, psychological, and spiritual conditions. The patient's health problems were identified as both physical and psychological. The physical condition deteriorated rapidly due to chronic pain and ineffective breathing related to pleural effusion on the right side.The psychological concern was added due to anticipatory grief of separation from family and the coming death.The process was a case of emotion with depression and sadness, resulting in the whole family falling into sadness. The author was able to offer empathy, emotional support and companionship to alleviate the patient's suffer and to provide emotional comfort to the family members of the patient in order to gain comfort and dignity for the case, and provide professional knowledge and ability for clinical nurses. The four core values of hospice care, including whole person, family-centered, continued care, and whole team, were applied in the care of the patient in order to relieve the patient's discomfort. Furthermore, teaching terminal care to his family so that the patient was able to accept being in terminal stage helps to achieve the goals of physiological, psychological and spiritual peace. |
