中文摘要 | 背景 : 親密伴侶暴力是全世界重要的公共衛生議題,其對個人、家庭與社區造成的危害不容忽視。 目的 : 探討 1997~2008年 18歲以上成人遭受親密伴侶暴力住院的傷害類型、住院醫療相關特性。 方法 : 使用 1997~2008年全民健保資料庫,串連「住院醫療費用清單明細檔」與「醫事機構基本資料檔」。選取 ICD-9 CM中 N Code為 995.8x與 E Code為 E967.3的受虐個案,並以 SPSS 18.0統計套裝軟體統進行分析。 結果 : 1997~2008年遭受親密伴侶暴力住院者計 464人,女性為男性的 32倍;平均住院天數為 4.53天,平均住院費用約為 17,159.89元。受虐者平均每人約有 2.6個傷害部位,傷害類型以顱內損傷比例最高(45.0%)。受虐患者多至區域醫院就醫(49.1%),25-44歲年齡族群住院比例最高(59.8%)。整體親密伴侶暴力住院率顯著上升,自 2004年到 2008年男性住院率增幅 300%(為女性的 15倍)。 結論 : 女性為遭受親密伴侶暴力的主要族群,但男性增幅迅速。衛生醫療體系應與其他專業資源整合,落實責任通報與轉介,提供持續性與整體性的全人照護,減少暴力傷害及預防再次受暴。 |
英文摘要 | Background: Intimate partner violence (IPV) is a global public health problem affecting individuals, families and communities. Purpose: Explore injury types and medical expenditures of inpatients above eighteen years of age admitted due to IPV between 1997 and 2008. Methods: This research used SPSS 18.0 software to analyze data from the Inpatient Expenditures by Admissions (DD) and Registry for Contracted Medical Facilities (HOSB) sections of the National Health Insurance Research Database. We defined intimate partner violence cases in accordance with ICD-9-CM N Code 995.8x and E967.3. Results: The study identified 464 inpatient cases resulting from IPV. Female IPV admissions were 32 times higher than male admissions. Mean of length of stay was 4.53 days and medical expenditures averaged NT$17,159 per subject. Subjects had an average of 2.6 injury sites of which intracranial injuries were most prevalent (45.0%). Among IPV inpatients, 49.1% were hospitalized in regional hospitals. The age group of 25-44 recorded the highest hospitalization rate (59.8%). IPV hospitalization rates revealed a rising trend over the research period, during which male admission rates increase 300% (15 times that of female admissions) between 2004-2008. Conclusions: Study findings indicate females as the highest risk group of intimate partner violence. However, male victims are growing rapidly in number terms. The healthcare system should provide integrated services, cooperate with other professional units, and refer patients to relevant authorities for help. Preventing intimate partner violence is an important future priority for Taiwan. |
