英文摘要 | Microsurgical reconstruction around dependent part of ankle and heel is a technique demanding procedure. How to prevent pressure ulcer and facilitate the ease of post-op flap care are very important. We designed a modified Hoffmann E.S.F. apparatus, which added 4 extra downward extension rods on the original half-pin, double-plane framework. {Clinic study} 12 cases with soft tissue defect around ankle and heel were enrolled in our study. All of them were Gustilo's open type IIIb, c fractures. This modified Hoffmann E.S.F. was applied at the same time with free flap surgery. The average follow up was 18.2 months. {mechanical study}Both this modified frame and the classic half-pin double-plane Hoffmann E.S.F. were applied on 8 pairs of synthetic tibial bones at random. Various loading modes were tested on MTS machine. All these 12 patients obtained successful muscle flap reconstruction. All the skin grafted on these muscle flap headled uneventfully, and no deep infection was found. The nonunion rate was 8.3%. 75% of our cases got good functional outcomes. There was no dependent part of soft tissue necrosis, nor pressure ulcer. All the caring staffs were highly satisfied with this design because of its ease of care and change dressing. No any further suspension devise, nor elevation cushion needed. the MTS test showed no significant difference between the modified and classic E.S.F. frame in the mechanical stability (p>0.05). Base on our study, this modified Hoffmann E.S.F. apparatus is suitable for both the skeletal stability keeping and soft tissue handling in free soft tissue flap reconstruction around ankle and heel. We would like to introduce this simple and effective design to all our orthopedic surgeons. |
