中文摘要 | 目的:報告兩例眼梅毒以視網膜及視神經新生血管為表現的病例。方法:病例報告。結果:兩位皆為57歲女性。第一位病患主訴雙眼視力模糊,第二位病患因右眼視力模糊並疼痛來門診就醫。兩位患者經由血清檢查診斷為眼部梅毒並接受神經梅毒的治療(靜脈注射aqueous penicillin G)合併玻璃體內bevacizumab注射,治療後,梅毒血清指數下降且視力改善。結論:以視網膜及視神經新生血管為表現的眼部梅毒患者以神經梅毒的治療方式合併玻璃體內bevacizumab注射,在此兩例病例中,可達到不錯的治療效果。 |
英文摘要 | Purpose: To report two cases of ocular syphilis presented with retinal and disc neovascularization. Method: Two case reports Result: The two cases are both 57 y/o females. Case 1 presented with blurred vision (OU) for 1-2 months. Fundoscopic examination revealed optic discs edema (OU), macular edema(OU) and a neovascular membrane inferior to macula(OS). Case 2 came with blurred vision and ocular pain (OD) with headache for 3 weeks. Panuveitis with uveitic glaucoma (OD), posterior uveitis (OS) were impressed after ophthalmic examination. Fluorescein angiography (FAG) revealed retinal and disc neovascularization in both cases. Laboratory workup showed elevated rapid plasma reagin (RPR) and T. pallidum hemagglutination assay (TPHA) titers in both cases. They received neurosyphilis treatment with intravenous penicillin G. Due to persisted retinal neovascularization and macular edema, intravitreal bevacizumab was given to both patients. Optical Coherence Tomography (OCT) showed resolution of macular edema and their visual acuity improved in the later follow-up visits. Conclusion: Treatment of ocular syphilis with neurosyphilis treatment protocol combined with intravitreal bevacizumab injection appeared to be effective in patients presented with retinal and disc neovascularization. |
