英文摘要 | BACKGROUND: Tonsillectomy is one of the most common surgical procedures in otolaryngology. However, few reports in the English medical literature and no reports in the medical literature of Taiwan compare the results of tonsillectomy performed under local and general anesthesia. The aim of the present study is to provide some objective information regarding tonsillectomy under local anesthesia versus general anesthesia. METHODS: The medical records of 321 patients who received bilateral tonsillectomy from July 2004 to June 2007 were retrospectively reviewed and clinical data (including operating time, blood loss, number of hospital days, cost during hospitalization, and postoperative complications such as postoperative hemorrhage and pain) were collected and analyzed. The operation was performed under local anesthesia in 234 patients and under general anesthesia in 87. RESULTS: Under local anesthesia, the operation required less time to perform, produced less intraoperative bleeding, resulted in fewer hospital days, and cost less compared with the operation under general anesthesia. There was no significant between-group difference in the amount of hemorrhaging and pain on post-operative days 1, 3, and 7. CONCLUSION: Tonsillectomy under local anesthesia is cost-effective and safe, produces less intraoperative blood loss, and is associated with a lower incidence of complications and therefore should be used preferentially in low gag reflex and cooperative teenage and adult patients. |
