中文摘要 | 臨床對比增強磁振血管造影(CE-MRA)檢查常使用三維-飛行時間(3D-TOF)波序。對比增強磁振血管造影利用顯影劑Gd-DTPA之T1縮短性質,甚至使得血管遠端分支亦清晰可見。然而往常在使用0.2 mmol/kg Gd-DTPA濃度之臨床例行檢查時,有時可能會獲得品質差的血管影像,造成診斷困難。此研究之目的在於評估臨床例行對比增強磁振血管造影檢查,以三維-飛行時間波序掃描及NV陣列線圈之推薦劑量。此研究包括兩部分。假體研究部分使用兩套10支試管,分別內含不同劑量Gd-DTPA及Gd-DTPA-BMA的生理食鹽水溶液。研究發現使用三維-飛行時間波序掃描Gd-DTPA及Gd-DTPA-BMA假體所得之最佳劑量爲20mM。發現使用Omniscan Gd-DTPA-BMA 及Magnivist Gd-DTPA所得之最佳劑量相同。在臨床評估部分,所有採用0.1 mmol/kg 30cc及3~4 cc/sec注射速率之臨床例行對比增強磁振血管造影檢查案例均可獲得高品質磁振血管影像。此劑量及注射速率於本院已沿用數年,且均能一致地得到品質良好的血管影像。此研究可提供例行對比增強磁振血管造影檢查Gd-DTPA之推薦劑量,好處爲可降低Gd-DTPA劑量及其潛在毒性,進而亦可降低顯影劑之成本。 |
英文摘要 | Three-dimensional time of flight (3D-TOF) pulse sequence is commonly used in clinical magnetic resonance angiography (MRA) examinations. Contrast-enhanced magnetic resonance angiography (CE-MRA) takes the advantage of the T1 shortening property of contrast agents Gd-DTPA, so that even the distal branches of vessels can be clearly visualized. However, the routinely used Gd-DTPA with 0.2 mmol/kg concentration might sometimes get poor quality of angiographic images which make the diagnosis difficult. The purpose of this study is to evaluate the appropriate dose of 3D-TOF pulse sequences used in routine CE-MRA examinations. Phantom and clinical evaluations were included in this study. In the phantom study, two sets of ten test tubes were filled with saline doped with different concentrations of Gd-DTPA and Gd-DTPA-BMA. It was found that the optimal dose was 20 mM for both Gd-DTPA and Gd-DTPA-BMA doped saline phantoms by using 3D-TOF pulse sequences and neurovascular (NV) array coil. There was no difference of the optimal doses between Omniscan Gd-DTPA-BMA and Magnevist Gd-DTPA contrast agents. In the clinical evaluations, it was found that consistently high quality CE-MRA images could always be obtained by using 0.1 mmol/kg Gd-DTPA along with 30 cc injection and 3~4 cc/sec injection rate. In conclusion, we propose an effective protocol for CE-MRA examination. The benefits of this protocol might be to minimize dose and potential toxicity. Additionally, the decrease of the cost of contrast agents might be achieved. It is expected to provide the recommended dose of Gd-DTPA for CE-MRA in clinical routine diagnosis. |
