中文摘要 | 鼻咽癌病患最主要的治療方法是放射線治療,但放射治療可能會傷害照野中的組織,理論上有致癌的可能性,對於照野外之影響相關之討論很少。透過治療儀器與治療計畫的進步,近代的放射治療確實改善了腫瘤的劑量分佈,減少照野中重要器官的劑量,但對於照野外的器官組織並未加以重視,因而可能增加了許多正常組織的不必要劑量。本研究以擬人假體分別模擬女性與男性鼻咽癌病人,在給予腫瘤劑量7000 cGy分35次照射的條件下,利用螺旋斷層治療機進行放射治療。以熱發光劑量計(TLD-100)度量出放射治療時的乳房劑量,並以2007年ICRP No. 103報告所提出的標稱危險度係數,評估出乳房二次致癌的危險度。本研究所測得的女性乳房劑量為19.46 cGy、男性乳房劑量為18.40 cGy。乳房二次癌症危險度在全年齡為2.2×10^(-3)。男性乳房二次癌症危險度在全年齡為2.1×10^(-3)。本研究發現螺旋斷層治療機對放射治療頭頸部的病人而言,照野外乳房劑量相較於其他治療方法為低。 |
英文摘要 | The main treatment of nasopharyngeal cancer (NPC) was radiotherapy. Irradiation might cause normal tissues damage in the radiation field. Recently, radiotherapeutic equipments and techniques were progressed more precisely, and caused to reduce side effect, but the organs and tissues out of fields also received scatter and leakage dose during radiotherapy. This study was designed to use an anthropomorphic phantom as different gender NPC patients with or without breast tissue with helical tomotherapy. The tumor dose was prescribed as 7000 cGy in 35 fractions. TLD-100 chips used to measure breast doses. The nominal risk coefficients of ICRP No. 103 was used to calculate the risks of second breast cancer. The measure dose of female breast was 19.46 cGy. The measure dose of male breast was 18.40 cGy. The second cancer risk of breast in female NPC patient was 2.2×10^(-3), in male NPC patient was 2.1×10^(-3). For head and neck cancer raditherapy in our study, the lower extra target doses were found in helical tomotherapy. |
