中文摘要 | 前牙錯咬為兒童早期混合齒列時期的一種不正咬合,約占咬合不正比例4-5%。造成此異常咬合的可能性很多:可能是局部的齒源性原因(後天外傷、多生牙或牙瘤的推擠),或是因先天下顎骨的過度生長所導致。若是齒源性之前牙錯咬須早期矯正,目前臨床上採用的治療方法亦不少,包括有:壓克力斜面咬合板、樹脂斜面、活動式矯正器與固定式矯正器⋯等。本病例旨在於報告:兩名七歲、八歲前牙齒性錯咬的患童,其前牙垂直覆蓋深度(overbite)分別為1.5釐米以及4.5釐米,分析個別差異後,兩名患童分別以壓克力斜面咬合板及固定式矯正器作治療,治療期間患童配合態度良好,於兩週及一個月後均獲得咬合改善。因此本次討論將著重於:臨床上如何針對具不同前牙垂直覆蓋深度(overbite)的患童,有效地選擇合適的矯治方式,而能於最短的治療時程內矯正完成並存有良好的穩定性。若能於此黃金時期以合適的方式矯正此種異常咬合,不僅能維護患童牙周、齒質及顳顎關節的健康,對於其日後口腔、臉部的發育亦有正向的助益。 |
英文摘要 | Anterior crossbite is one of the malocclusion during early mixed dentition period, and it also accounted for 4-5%. There are many causes leading to anterior dental crossbite: maybe due to local odontogenic factors (ie, trauma、supernumerary teeth and odontoma); it may also be caused by overgrowth of lower jaw bone. It is necessary to correct this situation in early mixed dentition period and there were variety of treatment options proposed, including: IBP Composite resin slope removable orthodontic appliance and fixed orthodontic appliance. Two 7 and 8 y o little patient diagnosed with anterior dental crossbite with 1.5mm and 4.5mm overbite respectively received treatment: the former was treated with IB1 the latter was treated with fixed orthodontic appliance. They were both cooperative and achieved normal occlusal relationship after 2 weeks and 4 weeks later respectively. In this case report, we will focus on: how to choose appropriate clinical methods for such cases with shallow and deep overbite and also worked efficiently, obtained good retention after treatment finished. If we can choose appropriate treatment option in this golden stage (early mixed dentition period; it may not only protect patient, s periodontal tissue tooth structure and TMJ, but also be beneficial to patient, s following orafacial development. |
