中文摘要 | 癌症為 10大死因第一名,其中直腸結腸癌發生率逐年的增加中。主要治療為手術、放射線治療、化學藥物治療,依照 NCCN的治療原則,術前可先使用 5-FU合併放射線療法或 5-FU合併 leucovorin/RT或capecitabine(Xeloda)/RT,經腹部手術後再使用 5-FU ±leucovorin或FOLFOX或Xeloda;若不適合藥物治療的病患,可經手術後再使用藥物治療。 Oxaliplatin適應症為治療第三期的結腸癌或直腸癌,可合併使用 5-FU及 leucovorin。建議劑量為成人每 2週靜脈輸注 85 mg/m2或每 3週連續使用 5天,每次 20~25 mg/m2或每 2~3週使用 100~130 mg/m2。過敏反應發生率 <1%但易於數分鐘內發生,症狀為疹子、蕁麻疹、紅斑、癢,嚴重會支氣管痙攣及低血壓。若發生時可先行停藥,使用抗組織胺藥物治療,嚴重時可使用 epinephrine、點滴輸注、 EKG監測、呼吸道的照護等。 |
英文摘要 | Cancer is the top 1 leading causes of death in Taiwan.The incidence of colorectal cancer is increasing year by year. Treatments of colorectal cancer included surgery, radiotherapy and chemotherapy. According to the guideline of NCCN, 5-FU in combination with radiotherapy or 5-FU in combination with leucovorin/RT or capecitabine/RT is administered prior to surgery. 5-FU combined with or without leucovorin or FOLFOX or xeloda is used after the abdominal surgery. Those patients who were unsuitable to drug therapy were treated with surgery in advance, then followed by drug therapy. Oxaliplatin for stage III colorectal cancer, may combined with 5-FU/leucovorin The recommended adult dose is 85 mg/m2 IV infusion eyery 2 weeks or 20-25 mg/ m2/day IV infusion for 5 days every 3 weeks or 100-130 mg/m2 every 2-3 weeks . The incidence of anaphylactic reactions is rare(<1%), but occured within minutes if happened. The signs of allergic reaction with oxaliplatin include rash, urticaria, erythema and itching. Sever allergic reactions would result in bronchospasm and hypotension. Oxaliplatin therapy may be discontinued if these allergic reactions occurred, and managed with antihistamine, epinephrine, EKG monitoring and air way supporting. |
