中文摘要 | 本專案旨在提升加護病房病患轉出物品交班之完整性,以減少延誤病患的治療和降低護理人員之負擔。本加護病房病患轉出物品交班之完整率僅40%,經現況調查發現原因爲缺乏輔助查核單張、護理人員對轉床作業認知不足、查核物品不確實、缺乏病患轉出物品交班之標準流程、物品放置分散未統一擺放及對病患物品不清楚。經2008年3月6日至5月22日應用設計病患轉出物品檢核表、宣導藥車/餐車轉發時間、制訂加護病房病患轉出物品交班之標準作業流程、推行"5S"運動、運用物品置物籃及落實於交班卡上註明病患私人物品後,病患轉出物品之完整率由40%提升至87%,使護理人員可增加直接照護病患之時間及病患安全,進而提升護理品質。 |
英文摘要 | The aim of this project was to increase the completion rate for the transit of belongings of intensive care unit (ICU) patients during a duty shift. If successful, this would reduce treatment delays and the burden on nursing staff. At the start of this study, the completion rate for moving ICU patient belongings during a duty shift was only 40%. We found that delays were incurred due to demanding supplementary checklists, the lack of cognition of the patient undergoing transfer, inaccurately counting of items, a need for a standard operating procedure (SOP) for the process, a failure to put away scattered items and uncertainty about what were the patient's belongings. From March 6(superscript th) to May 22(superscript nd), 2008, we created and started to use a better patient belonging checklist, identified a designated time for the carts to carry medication, food and other items between the ICU and the wards, instituted a SOP, boosted the "5S" movement, introduced storage baskets for patient belongings and created a written log of patient belongings on cards. These improvements increased the completion rate from 40% to 87%. As a consequence, the available time for direct patient care of patients was increased, patient safety was improved and, overall, the quality of care in the ICU was enhanced. |
