中文摘要 | 本院原設立一位出院準備服務個案管理專責人員,經分析發現執行面多有疏漏之處,因而引發此專案動機。目的為應用個案管理模式在單位內設立種子人員整合各項資源,以合適的篩選工具,儘早評估個案出院照護需求,運用團隊力量進行多方面討論,協助其順利出院。專案過程以準備期、執行期、評值期進行,透過各病房選立一位出院準備服務種子人員並安排培訓課程,制定個案篩選登錄方式、修訂出院準備服務作業流程,以及舉辦與出院準備服務相關之在職教育等策略。實施後發現人員篩選執行能力提昇,對出院準備服務內容認知程度改善,發揮團隊連結的力量達到個案管理的成效。出院準備篩選執行率由3.4%增至95.0%,人員的認知程度由56.4%增至93.1%,病患滿意度由78%增至95%。由此專案結果建議全院各病房積極推動種子人員運用個案管理模式提供出院準備服務,提昇服務效率與品質。 |
英文摘要 | There were some flaws that only one case manager to serve the discharge planning in the hospital. The goals of "seed-instructor" program was to improve case management for discharge planning. The Seed Instructor of case management in the ward can integrate various resources, using the suitable screening technique evaluate the patient's care plans after discharge as soon as possible, discussing with team members and to help them discharged successfully. The preparation, implement and evaluation stage of the program, include electing a nurse to be "seed-instructor" and arranging active training courses, promoting appropriate case registration, and revising the standard procedures of the discharge planning. The results of this project showed that execute screening rate of discharge planning, staff's perception of the program, and patient satisfaction increased from 3.4% to 95.0%, from 56.4% to 93.1%, and from 78% to 95%, respectively. The staff's ability of executing case selection and of recognizing the discharge planning context was improved, so the case management system becomes much more effective by group dynamics. In conclusion, the implementation of "seed-instructor" program in every ward by making use of the case management system will promote both efficiency and quality of the discharge planning. |
