| 中文摘要 | 臨床操作能力評核(PBA)是以臨床操作手術為中心基礎(procedure based),兼顧技術(technical), 策略流程(operative),與專業素養(professional)等三項臨床能力評估。臨床操作能力評核(PBA)的構面(domains)包括:知情同意(informed consent),術前計畫(pre-operative planning),術前準備(pre-perative preparation),手術野展露與閉合(exposure and closure),手術中技巧(intra-operative techniques),以及手術後的處置(post-operative managements)等六項。因應醫學教育改革走向實務化,延伸至工作場,知識技能與態度並重,以及主動,互動回饋及終身學習的概念,臨床操作能力評核(PBA)正好具有這種特色。 與直接觀察操作技巧評估(DOPS)只作操作能力概括性評估不同的是,臨床操作能力評核(PBA)之特質,尤其適用於次專科訓練不同操作或手術為中心的病人照顧評核。這種評核工具起始於英國,但目前尚屬萌芽階段,亞洲國家至今則尚未有相關的經驗發表。本文作者參考英國原表的精神,融入全人照顧的觀念,加入華人社會的特色以及考量與DOPS間的互變相容性,改寫成全新中文化的表格。在台灣雲嘉地區兩家大型區域教學醫院(各近千床容額)推展實施, 其相關結果也在新加坡亞太醫學教育學會作英語海報口述發表。今特將敝人發展表格及推行經驗,以及初步成果在此發表。基於醫學教育未來改革的潮流,臨床操作能力評核(PBA)必將於未來發揮更重要的角色。 |
| 英文摘要 | Many procedure skills assessment tools have been implemented in the medical students world-wide. However, little studies about the assessment of surgical skills have ever been reported in the Asian-Pacific medical institutes. In this study, we developed our own learning systems and assessment tools of surgical skills which is modified from previous Euro-American well-known medical centers. The dimensions of assessment include the informed consent, pre-op preparation, pre-op evaluation, operative skills and post-operative care. The place of assessment is at the workplace (ward or operating room). The scoring system is modified as 9-grade scales. We assess our students after creating this system at the website. We used repeated measures for certain students to evaluate their performance improvement. Feedback will return to the students for future behavior modification. The detailed of results will be presented here. This assessment has been implemented in two large teaching hospitals (totally about 2,000 Beds) in Chia-Yi and its preliminary results have been exhibited in poster in 2009 Asia-Pacific Medical Education Conference (APMEC) held in National University of Singapore. We believe that clinical skill-centered, multidimensional competency assessment in work-place will play more important role in the future medical education reform. |
