中文摘要 | 本文係在探討一位口腔癌末期合併腫瘤潰瘍個案的身、心、靈健康問題,筆者透過身體評估、會談、觀察家庭互動等方式,以Gordon十一貢健康功能模式評估個案的身、心、社會、靈性等層面的健康問題,確立個案有組織完整性受損、慢性疼痛、身體心像紊亂、心靈困擾等健康問題。護理過程中筆者藉由完整的傷口評估,提供個案傷口照護與舒適謢理技巧,以減少傷口滲液及惡臭,並減輕個案因傷口所產生的困窘及羞愧感;更運用藥物及非傳統止痛藥物方式,減輕個案的疼痛不適;同時藉由關懷、傾聽、同理、與接納,使個案能夠正向的接受身體結構功能的改變;此外,藉由安寧醫療團隊社工師及宗教師的介入,協助個案明白佛教的真理,並使其勇敢地面對死亡,期許此篇個案報告能夠提供安寧護理人員於協助個案往生過程中,如何運用靈性的力量來克服所面臨的死亡焦慮,及讓個案能夠做好自己的死亡準備,使個案及家屬們能平安且寧靜地面對此死亡的過程,達到雙贏目標的參考。 |
英文摘要 | This case explored the physical, psychological, and spiritual problems of a patient with oral cancer and tumor ulcer at the terminal stage. The authors assessed the patient’s physical, psychological, social, and spiritual problems based on Gorden’s eleven health functional patterns through physical assessment, interviews, and observation of family interactions. Four patient’s problems were identified: tissue integrity, impairs; chronic pain; body image, disturbed; spiritual distress. During the nursing process, the authors assessed the wound thoroughly, offered wound care and comfort skills, and reduced the ooze and stench of the wound to reduce the patient’s feeling of embarrassment and shame. In addition, the authors relieved the patient’s pain through using the pharmacological and non-traditional pain-reveling strategies. The patient finally could positively accept the changes of this physical structures and functions. Furthermore, we assisted the patient to understand the truth of Buddhism and bravely face the death through involving the social worker and the religionist of the health care team into the hospice care. It is expected this case report can offer nurses in the hospice units how to apply the strength of spirituality to overcoming the patient’s anxiety for death during the dying course. Nurses can help dying patients make good preparations for this own death. Both the patient and the family can face the dying process peacefully and serenely. Nursing care can achieve win-win situation. |
