中文摘要 | 背景:鑑於全國性之急診專科醫師人力供需現況調查仍付之闕如,本研究之目的為了解我國急診專科醫師人力及分佈'以作為政府制訂專科醫師人力規劃政策之參考依據。方法:以2003和2006年內政部之人口統計數及行政院衛生署之全國急診就診人數,2003年及2006年各縣市衛生局 執業登錄為急診醫學科及其執業醫院為地區醫院以上之醫師,及急診醫學會之會員基本資料檔來了解2003年及2006年台灣不同地區之急診服務量分佈情形,急診專科醫師人力結構特徵及地理分佈現況。結果:2003年台灣地區有629位急診專科醫師,但卻只有605位留在急診室執業服務,且有一半以上急診專科醫師 集中於台北市、桃園縣、台中市及高雄市,有7個縣市少於10人,分別為金門縣、連江縣0人、澎湖縣2人、雲林縣3人、台東縣4人,苗栗縣6人及新竹縣9人,至2006年台灣地區現有急診專科醫師955位,卻只剩下533位現仍在急診執業服務,且有一半以上執業急診專科醫師集中於台北市、桃園縣、台北縣、臺中市、高雄市及彰化縣,仍有7個縣市少於10人,分別為金門縣、連江縣0人,澎湖縣3人,台東縣5人,新竹縣7人,苗栗縣7人及基隆市8人。結論:自2003年到2006年台灣地區每年約培養出100位急診專科醫師,但留在急診執業服務的急診專科醫師卻從2003年的605位到2006年只剩下533位,台灣急診專科醫師不僅具有明顯的高離退率,且明顯不足,並面臨嚴重分佈不均,尤以離島縣市更是嚴重。政府當局不僅應訂定獎勵辦法鼓勵急診專科醫師到缺乏縣市服務,更應保障急診專科醫師的工作環境,降低離退率,以期留住培養不易的急診專科醫師,為急症患者做最好的服務。 |
英文摘要 | Background: The nationwide investigation for the emergency physician(EP) manpower supply and demand study is lacking in Taiwan. The purpose of this study is to understand the current EP manpower situation and distribution in order to be as the government reference for the EP manpower plan policy. Methods: Data were collected to understand the emergency visits and EP manpower distribution in 2003 and 2006 in Taiwan. Sources for data collected included 1) the total nationwide emergency department visits in 2003 and 2006 according to the data base of Department of Health 2) the number of Board certified EP and their working emergency department according to the biographical information from the Taiwan Society of Emergency Medicine in 2003 and 2006. Results: There was 629 EP at the end of 2003 in Taiwan ,but only 605 EP worked in the emergency department. More than one-half of the 605 EP allocated to Taipei City,Tao Yuan County,Tai Chung City and kaohsing City. But with <10 EP in the following districts : Kinmen County and Lienkiang County(0), Penghu County(2),Yunlin County(3), Taitung County(4), Miaoli County(6), Shinchu County (9) . In 2006, there was 955 EP in Taiwan but only 533 EP work in the emergency room at the present time. More than one-half of the 533 EP allocated to major cities (i.e., Taipei City, Tao Yuan County, Taipei County,Tai Chung City, Kaohsiung City and Changhua County) and with < 10 EP in the following districts: Kinmen County and Lienkiang County(0), Penghu County (3), ),Taitung County(5), Miaoli County(7), Shinchu County (7) and Keelung City(8). Conclusions: There are about 100 EP trained every year since 2003 to 2006. So, there were 629 board certified EP in 2003 and 955 in 2006, but only 605 board certified EP in 2003 and 533 in 2006 work in the emergency department at the present time, indicative of a shortage of emergency medicine personnel resources and a high EP retired rate. However, the maldistribution of EP is expected to persist in offshore cities and counties. The uneven distribution is a common problem in Taiwan, it need good policy of Department of Health to improve this condition. A suitable working environment for EP is also needed to decrease the retired rate. |
