中文摘要 | 背景:臨床上會干擾分析的物質很多,也很難將它們一一測試出來。在放射免疫分析實驗室的試劑中,有些廠商將溶血、黃疸和血脂箏因素列爲操作上的限制,有些則無任何註記。爲了探究檢體溶血對於放射免疫分析法的影響情形,我們將同一位息者的血液檢體分為無溶血和溶血兩組,觀察溶血對於CA 125檢驗結果的影響情形。方法:本研究採用放射免疫分析法ELSA-CA 125 Ⅱ同時進行溶血與無溶血檢體之血清CA 125抗原濃度測定,再將結果以統計方法分析,評估溶血檢體是否會干擾CA 125的测定。結果:溶血和無溶血兩組之CA 125檢驗結果在統計上雖有差異,但在臨床上並無重要影響。討論:若溶血檢體在放射免疫分析法的偏差值可被接受,且不影響醫師在臨床上病情評估,不但可減輕病患挨針之苦與路途奔波之勞累,減少檢體退件的繁雜作業與件數,更可縮短報告的作業時間。 |
英文摘要 | Background: In a medical laboratory, many factors may affect the analytic results and lead to a clinical dilemma. Some limitations from the procedures, described by the manufactory of the analytic kits, mentioned that when serum samples present with turbidity, hemolysis, hyperlipemia or fibrin, it may come out misleading results. In this study, we explore whether the hemolyzation impact the results of radioimmunoassay study or not. Methods: Using CA 125 as the study target, serum samples were collected and divided into hemolyzed or non-hemolyzed groups. Hemolyzed group was constructed from non-hemolyzed samples using mechanical methods. The Wilcoxon Signed Rank test was applied for testing the statistical significant difference and the Bland-Altman analysis was used for clinically importance evaluation. Results: The Wilcoxon Signed Rank test showed statistical significant difference between the hemolyzed and non-hemolyzed groups. However, in the results of Bland-Altman analysis, no clinically importance between these two groups was demonstrated. Conclusion: No definite clinical difference in radioimmunoassay analysis was found between hemolyzed and non-hemolyzed samples. It means that when patients have difficulties for re-sampling their serum, the data still can be used clinically. |
