中文摘要 | 過敏性氣喘(Allergic Asthma)為臨床最常見小兒過敏性疾病之一。本實驗以Dunkin-Hartley天竺鼠1O8隻,利用天竺鼠氣喘模式來探討氣喘緩解期,在中醫治則上緩者治其本 (腎 ) 之原則下投與腎氣丸。探討腎氣丸對氣喘天竺鼠發炎細胞(嗜中性白血球、嗜酸性自血球、巨噬細胞、淋巴球)與炎症介質(LTC4、PGE2)之影響及肺部病理變化。我們利用單因子變異數分析(one way ANOVA)比較各組間的差異。其結果顯示:一、乙醯膽鹼激發試驗(Acetylcholine provocation test);氣喘天竺鼠服用腎氣丸後,引起支氣管收縮之乙醯膽鹼濃度明顯高於對照組。二、肺泡沖洗液發炎細胞總數與比例分析:腎氣丸組巨噬細胞總數在1、6、24、48小時顯著降低,嗜中性自血球、嗜酸性自血球、淋巴球則無顯著差異。三、肺泡沖洗液之前列腺素E2 (Prostaglandin E2)分析:腎氣丸組在1小時顯著高於對照組。自三稀素(Leukotriene C4)分析:腎氣丸組在1、24小時顯著低於對照組。四、肺泡沖洗液之流式細胞計數儀分析:腎氣丸組CD3、CD4、 CD8 T淋巴球在72小時,CD8 T淋巴球在1小時有顯著高於對照組。五、肺組織病理切片檢查:腎氣丸組較對照組在肺泡間質嗜酸性自血球及嗜中性自血球的浸潤有減少。綜合以上結果,氣喘天竺鼠緩解期服用腎氣丸可降低呼吸道過度反應性與肺泡沖洗液巨噬細胞浸潤,其詳細機轉有待進一步探討。 |
英文摘要 | Allergic asthma is one of the most common chronic diseases in the field of pediatrics in Taiwan. The 108 male Dunkin-Hartley guinea pigs, weighting 350-450 gm were used in this study. Guinea pigs were sensitized intraperitoneally 100 μ g/ml by Dermatophagoides pteronyssinus (DerP) at day 0' 7 and challenged aerosolizedly lmg/ml by DerP at day 21. Shen-Qi-Wan was administrated from day 25 to day 31 until the second challenge (day 32). Acetylcholine provocation test, bronchoalveolar analysis, pulmonary pathologic examination and one-way ANOVA were applied to evaluate the effect of Shen-Qi-Wan in asthmatic guinea pigs. The results showed: (1) Acetylcholine provocation test: The concentration of acetylcholine had significantly increased in treated group. (2) Brochoalveolarlavage analysis: <1> inflammation cells: The total number ofmacrophage had significantly decreased at 1, 6, 24 and 48 hour in treated group. <2> Inflammatory mediator: The concentration of PGE2 had significantly increased at I hour and LTC4 had decreased at 1, 24 hour in treated group. <3> T-lymphocyte: The numbers of CD3, CD4T lymphocyte at 72 hours and CD8 T lymphocyte at 1, 72 hour were significantly increased in treated group. (3) Pulmonary histopathological finding: The inflammation cell infiltration of respiratory tract had decreased in treated group. According to above results, we suggest Shen-Qi-Wan can decrease the respiratory hyperreactivity and prevent moderately asthma attack. |
