中文摘要 | 研究目的:本研究之目的在探討以鉺雅鉻雷射照射牙齒後再塗上溝隙封填劑,是否會提高溝隙封填劑之留置率。 材料與方法:從台中榮總就診之兒童病患中選取6至10歲之兒童35位,其4顆恆牙第一大臼齒已萌發,且未蛀牙者。以split mouth technique,將第一大臼齒分為左邊及右邊兩組,一邊之臼齒,施以一般之溝隙封填劑塗佈方法為對照組,另一邊之臼齒則先以鉺雅鉻雷射100mJ照射咬合面10秒,之後再塗佈溝隙封填劑為實驗組。每隔三個月追蹤一次,觀察溝隙封填劑是否脫落。溝隙封填劑脫落之情形分為四類:(1)Type 0:溝隙封填劑完整未脫落;(2)Type Ⅰ:部分脫落,脫落處<1/2;(3)Type Ⅱ:部分脫落,脫落處>1/2;(4)Type Ⅲ:溝隙封填劑完全脫落。結果以student-t test比較,P值為0.05。 結果:有35個兒童參與,總共有98顆牙齒。三個月追蹤時共有27個兒童回診(74顆第一大臼齒),結果顯示實驗組中溝隙封填劑完好未脫落者佔64.9%,完全脫落者佔18.9%。在對照組中溝隙封填劑,完好未脫落者佔62.2%,完全脫落者佔18.9%。六個月追蹤時共有20個兒童回診(58顆牙),結果顯示實驗組中溝隙封填劑完好未脫落者佔51.7%。在對照組中溝隙封填劑完好未脫落者也佔51.7%。以student-t test分析,發現實驗組與對照組無論在三個月或六個月時之溝隙封填劑留置率,都沒有明顯之差異存在。 結論:雖然已有很多研究探討雷射處理對溝隙封填劑留置率之影響,但本研究是目前唯一之口內人體研究。本研究結果雖顯示牙齒表面經雷射處理後,在六個月追蹤時對溝隙封填劑之留置率並沒有明顯改變,但或許更長期之追蹤會有不同的結果。 |
英文摘要 | Purpose: The purpose of this study was to assess the retention rate of pit and fissure sealant following Er: YAG laser irradiation in vivo and compare it to conventional sealant application. Methods: Thirty-eight children aged between 6-10 years old were selected. These children had four erupted non-carious first permanent molars. Split mouth technique was used. Acid etching, bonding, and sealant application were performed on the first permanent molars of control side. Er: YAG laser irradiation for 10 seconds before sealant application was performed on the study side. Children were followed at three months and six months after sealant application. Sealant retention was categorized into four types: (1) type 0: complete retention, (2) type 1: sealant loss less than 1/2, (3) type 2: sealant loss more than 1/2, and (4) type 3: complete loss of sealant. Results: A total of 98 permanent molars were collected. In 3-month follow-ups, 74 molars were examined, 64.9% of sealants were completely retained in laser group and 62.2% were completely retained in control group. In six-month follow-ups, 58 molars were examined, 51.7% of sealants were completely retained in both laser and control groups. There was no significant difference between these two groups at both 3 and 6 months. Conclusion: This is the only in vivo study to investigate the laser effect on sealant retention up to now. Although there is no significant difference between laser and control groups at six-month follow-ups, longer follow-up time may show different results. |
