中文摘要 | 根據美國糖尿病協會的建議,使用筆型胰島素注射器時,不贊成重覆使用針頭 。我們報告兩個第1 型糖尿病患者,從未重覆使用針頭,卻在胰島素卡匣內出現紅血球沉積的案例。針對胰島素卡匣的使用之研究顯示,每一次的注射都可能造成針頭內或卡匣內殘存有被注射者皮膚的表皮細胞,且罹病年愈久者,在卡匣內及針頭內出現表皮細胞的機率也增加。至於在針頭或卡匣內出現紅血球細胞的情況則極少報導,推究其原因可能與注射前後,針頭內外的壓力差有關,也可能與重覆使用針頭的次數有關。至於把這些紅血球的沉積打入皮下,對於人體是否有影響並不清楚。所以我們建議注射後保持拇指按壓至抽離注射部位,之後立即將針頭拔除,以減少胰島素卡匣受到污染的機會。 |
英文摘要 | According to the recommendations of the American Diabetes Association, needles for insulin pen injectors should not be reused. We reported the finding of red blood cell deposition in the insulin cartridges of two type 1 diabetic patients who never reused pen injector needles. Studies on the use of insulin cartridges have shown that epidermal cells may sometimes appear within the needle or the cartridge after each injection. The probability of finding the cells increases along with the duration of diabetes. However, the appearance of red blood cells in the needles or cartridges has rarely been reported. Putatively it may be attributed to the pressure gradient between the exterior and the interior of the needle after injection, and may be associated with the number of times the needle is reused. It is not clear whether injecting the red blood cell residue into the hypodermal layer causes any untoward effect. We therefore recommend applying pressure on the injection site with the thumb until after the needle has been withdrawn. The needle should be removed from the pen immediately afterwards to reduce the chance of contaminating the insulin cartridge. |
