中文摘要 | 碳-13尿素呼氣試驗(�CC-Urea breath test, �CC-UBT)非但不具侵襲性,而且無論在治療前或滅菌後都具有高度的敏感性及特異性;值得推廣。目前,此項檢查仍局限於少數醫院中,在專人監督下進行。為評估以碳-13尿素呼氣試驗法居家偵測(home-based �CC-UBT)幽門螺旋桿菌的可行性;本研究共邀請104位志願者參與。受邀者於領取解釋操作過程的圖文說明及藥劑器材後,在家中自行採集基準值及30分鐘共兩次之呼氣樣品;於翌日接受內視鏡檢查時交還試管,送至核研所分析。 我們是採用快速尿素�t試驗及modified Giemsa組織染色為判定幽門螺旋桿菌存在與否的標準。在37位幽門螺旋桿菌真陽性與57位真陰性的受邀者中,“home-based �CC-UBT”的敏感性、特異性、正預測值及負預測值分別為97.3%、91.1%、87.8%及98.1%,並有93.5%之準確度。僅有3位受邀者認為操作過程複雜,1 位因為呼氣量採集不足無法分析。 本研究結果顯示將碳-13尿素呼氣試驗應用於幽門螺旋桿菌之居家偵測非但可行;而且藉由這種“方便性”的增加,有助於建立�CC-UBT推廣的模式。為維持居家偵測的準確性,給予受試者充分指導是必需的。 |
英文摘要 | The �CC-urea breath test (�CC-UBT) is a non-invasive and reliable mean of diagnosing Helicobacter pylori (H. pylori) infection. To popularize its use, we studied the feasibility of having patients perform the test at home without supervision. One hundred and four patients (70 men and 34 women, median age 43.5 years) were recruited; 37 were H. pylori-positive and 57 H. pylori-negative according to the results of rapid urease test and modified Giemsa stains. The �CC-UBTs were performed at home by collecting a baseline and a 30 minutes post-dosing samples. Patients visited the hospital on the next day to return breath samples and undergo upper gastrointestinal endoscopy. Sensitivity, spefcificity, and positive and negative predictive values of the home-based �C C-UBTs were 97.3% (36/37), 91.1% (51/56), 87.8% (36/41) and 98.1% (51/52), respectively, with a diagnostic accuracy of 93.5%. Only three patients found the test complicated. One patient failed to collect enough breath samples for analysis. Our study suggests that having patients perform the �C C-UBT at home is feasible. A thorough instruction, however, is essential to maintain the accuracy of the test. |
