中文摘要 | 目的:應用ARIS(Architecture of Integrated Systems)流程管理方法論於醫療作業流程,經由實際應用的過程總結出一個參考模式,並且探討實際執行要領與理論内涵的差異。 方法:以個案研究方法,實際參與我國一家300床規模的區域醫院急診部門流程管理專案的執行。工具上應用ARIS流程管理方法論的架構對病患就診過程進行分析,並且應用ARIS 6.2軟體工具實際進行醫療作業流程模型的初步建構。資料的蒐集包含由實際專案進行過程中深入觀察、訪談、攝像與錄影所得之資料,及由醫療資訊系統所收集的數據資料。所蒐集的資料主要是以質化的方式進行分析與管理意涵的解讀。 結果:個案醫院之應用過程被完整記錄成為一個參考模式,並且與相關理論進行比較。個案醫院於臺灣醫療品質指標計畫(TQIP, Taiwan Quality Indicator Project)相關指標上大部份能顯示改善的成效,文中亦對部份成效不顯著的原因進行應用限制的探討。 結論:ARIS流程管理方法在醫院管理的適用性得到了初步驗證,然其應用上亦需考量一些限制。此方法論可進一步應用到一般門診部門、醫療臨床路徑、醫療資材管理等業務,亦可與無線射頻(RFID, Radio Frequency Identification)機制的應用進行整合。 |
英文摘要 | Objectives: This paper presents the application of the methodology ARIS (Architecture of Integrated Information Systems) to the clinical processes of a hospital. By analyzing the application case, this study will develop a reference model for the process-oriented hospital management in which some implications in terms of implementation and theoretical meanings can be studied. Methods: A case study was performed at the emergency medical department of a regional hospital with 300 clinical beds. In it, the framework of ARIS was applied to analyze the patient flows, and its software toolset ARIS 6.2 was used to support the process model construction. Data was collected by participatory observation, through interviews, by photographing from the clinical processes, as well as by the healthcare information systems. The collected data was analyzed using a qualitative and interpretative research strategy. Results: The application case was well described to be a reference model, and the evidences were compared with the related theories. The results show that the performance improvement refers to most of the TQIP (Taiwan Quality Indicator Project) indexes was positive. Based on the evidences, the application limit of the ARIS methodology to the hospital was presented. Conclusions: This application case validates the feasibility of the ARIS methodology to the hospital. The resulting model can be further used to the clinical departments, clinical pathways optimization, health resources management, and even the application of Radio Frequency Identification (RFID). |
