中文摘要 | 原發性皮質醛酮症(primary aldosteronism)是指以高血壓伴低鉀血症、血漿腎素活性降低、血漿和尿液的皮質醛酮濃度增高為特徵的臨床綜合表現,是次發性高血壓需要去鑑別診斷之一,而其在高血壓病患中的盛行率約為5-15%左右。經過五十多年,原發性皮質醛酮症被認為是稀有的疾病,現在卻發現其可能是高血壓中可治癒的最常見的原因。本篇文獻報告一個長期高血壓個案,因高血壓變的難以控制,併發現低血鉀和代謝性鹼中毒入院進行影像和生理檢查,發現是腎上腺腺瘤所造成的皮質醛酮分泌過高。最後經由內視鏡摘除手術後,高血壓臨床症狀和皮質醛酮恢復正常而且不需要高血壓藥物控制。由於在篩檢時很容易忽略皮質醛酮症引起的高血壓而漏掉診斷,使病人持續受頑固性高血壓之苦,所以特別提出本案例報告來提醒大家注意。 |
英文摘要 | Primary aldosteronism (PA) affects 5-15% of patients with hypertension, characterized by inappropriate production of aldosterone. Increasing evidence indicates that PA is much more prevalent than previously believed, making this disease the most frequent cause of secondary hypertension. The fact that PA can be specifically treated and potentially cured makes the diagnosis a particular challenge for clinicians. Finally, localizing procedures are necessary to differentiate among the various forms of PA to determine the best therapeutic approach, since only the unilateral form has consistently been shown to respond to surgical intervention. Herein, we present how a 47 years old female patient‚who had suffered from intractable hypertension and hypokalemia‚ was proved to have an aldosterone producing adenoma. Blood pressure turned normal after laparoscopic adrenalectomy. This case highlights the importance of working for aldosteronism in patients that develop abrupt hypertension. |
