英文摘要 | To assess the usefulness of electron beam computed tomography (EBCT) in estimating the McGoon ratio for predicting pulmonary flow. Over 18 months (1999 - 2001), 457 patients (mean age = 10 years; standard deviation = 14.9 years; 206 female and 251 male) received cardiac EBCT preoperatively to provide information for further management. The data were analyzed retrospectively. Detailed clinical evaluation, chest roentgenogram, electrocardiogram, two-dimensional and Doppler echocardiography, and cardiac catheterization with cineangiography were performed in all patients. Written consent was obtained from all subjects or guardians prior to EBCT. All EBCT images were taken at the end-diastolic phase of the cardiac cycle with intravenous injection of iodinated contrast medium. Cardiac catheterization was carried out using standard techniques. Pulmonary arteries and descending aorta were measured on different planes and EBCT McGoon ratio was calculated. The pulmonary arterial size and McGoon ratio determined by cardiac catheterization were used for comparison. Pearson’s correlation and student paired t test were used for statistical analysis. There is significant difference between EBCT and angiography in PA diameter as well as McGoon ratio measurement (P<0.001). High correlation is noted in the pulmonary arterial sizes and McGoon ratio on EBCT to the angiographic projections (Pearson’s correlation >0.9, and P<0.001). The calculated EBCT-McGoon ratio is a reliable estimate to the conventional McGoon ratio measured by angiography. Noninvasive EBCT with reconstructed oblique sagittal imaging is a promising estimate of pulmonary arterial size to calculate the McGoon ratio and pulmonary flow accurately. |
